经皮脊柱内镜腰椎间盘摘除术并发类脊髓高压综合征4例报道

摘　　要：因经皮脊柱内镜手术引起术中、术后类脊髓高压综合征是一种较为罕见的腰椎微创手术并发症,国内目前仅见个案报道。本文回顾性分析4例2014年1月至2021年12月于深圳市中医院行经皮脊柱内镜腰椎间盘摘除术并发类脊髓高压综合征患者的临床资料,结合文献进行总结。类脊髓高压综合征主要临床表现为头颈部突发且较为严重的疼痛、胸口憋闷感、耳鸣等,还可表现为极度烦躁、濒死感,逐渐出现患肢向健肢进展性麻木、腹胀、颅内压增高,或出现血压骤升、心率加快等体征。充分认识类脊髓高压综合征,选择合理的治疗措施,可有效缓解、预防该并发症。Intraoperative and postoperative myeloid hypertension-like syndrome caused by percutaneous spinal endoscopic surgery is a rare complication of minimally invasive lumbar spine surgery,and only case reports have been reported in China.This article retrospectively analyzes clinical data of four patients with percutaneous endoscopic lumbar discectomy complicated with myeloid hypertension-like syndrome in Shenzhen Traditional Chinese Medicine Hospital from January 2014 to December 2021,and summarize based on literature.Main clinical manifestations of myeloid hypertension-like syndrome are sudden and severe pain in head and neck,chest tightness and tinnitus,etc.It can also manifest as extreme restlessness,a sense of impending death,gradually developing progressive numbness from affected limb to healthy limb,abdominal distension,increased intracranial pressure,or signs of sudden increase in blood pressure,rapid heart rate,and other signs.Fully understanding myeloid hypertension-like syndrome and selecting reasonable treatment measures can effectively alleviate and prevent this complication.
