血府逐瘀汤联合血管腔内介入治疗下肢动脉硬化闭塞症的临床观察

摘　　要：目的探讨血府逐瘀汤联合血管腔内介入治疗下肢动脉硬化闭塞症的临床效果和安全性。方法收集2020年3月至2022年6月于北京中医药大学附属护国寺中医医院住院治疗的60例下肢动脉硬化闭塞症患者的临床资料,根据住院期间治疗方案的不同将患者分为联合组(n=28,采用血府逐瘀汤联合血管腔内介入治疗)和对照组(n=32,采用血管腔内介入治疗)。比较两组患者的手术部位,治疗策略,临床疗效,并发症发生情况,术前、术后1个月的足背动脉血流量、踝肱指数(ABI),以及术后3个月的血管通畅率。结果两组患者的手术部位、治疗策略及治疗后并发症的总发生率比较,差异均无统计学意义(P﹥0.05)。治疗后,联合组患者的总有效率高于对照组患者,(P=0.036)。两组患者术后1个月的足背动脉血流量和ABI均高于本组术前,且联合组患者术后1个月的足背动脉血流量和ABI均高于对照组患者(P﹤0.05)。联合组患者术后3个月的血管通畅率高于对照组患者(P﹤0.05)。结论血府逐瘀汤联合血管腔内介入治疗下肢动脉硬化闭塞症具有较好的临床效果与安全性,值得临床推广。Objective To investigate the clinical efficacy and safety of xuefu zhuyu decoction combined with endovascular intervention in the treatment of lower extremity arteriosclerosis obliterans.Method The clinical data of 60 patients with lower extremity arteriosclerosis obliterans who were hospitalized at Huguosi Hospital of Traditional Chinese Medicine Affiliated to Beijing University of Chinese Medicine from March 2020 to June 2022 were collected.The patients were divided into combination group(n=28,treated with xuefu zhuyu decoction combined with endovascular intervention therapy)and control group(n=32,treated with endovascular intervention therapy)according to different treatment plans during hospitalization.The surgical site,treatment strategy,clinical efficacy,incidence of complications,dorsalis pedis artery blood flow and ankle brachial index(ABI)before and 1 month after surgery,and vascular patency rate 3 months after surgery were compared between the two groups.Result There were no statistically significant differences in the surgical site,treatment strategy and the total incidence of complications after treatment between the two groups of patients(P>0.05).After treatment,the total effective rate of the combination group patients was higher than that of the control group patients(P=0.036).The dorsalis pedis artery blood flow and ABI of both groups at one month after surgery were higher than those before surgery,and the dorsalis pedis artery blood flow and ABI of the combination group of patients were higher than those of the control group(P<0.05).The vascular patency rate of the combination group patients was higher than that of the control group patients at 3 months after surgery(P<0.05).Conclusion Xuefu zhuyu decoction combined with endovascular interventional therapy for lower extremity arteriosclerosis obliterans has good clinical efficacy and safety,and is worth promoting in clinical practice.
