N-trap阻石篮联合输尿管镜治疗输尿管上段结石的倾向性评分匹配研究

摘　　要：目的探讨N-trap阻石篮联合输尿管镜钬激光碎石术治疗输尿管上段结石的有效性及安全性。方法选取2019年8月至2022年8月因输尿管上段结石在上海交通大学医学院附属仁济医院泌尿科行输尿管镜钬激光碎石术治疗的114例患者为研究对象,根据是否使用N-trap阻石篮分为N-trap组(n=50)和对照组(n=64)。采用倾向性评分匹配法比较两组结石清除率、结石上移率、平均手术时间、手术并发症,术后随访6~12个月,比较两组术后输尿管狭窄的发生率。结果经倾向性评分匹配法1∶1匹配后,筛选出N-trap组及对照组各45例,匹配分析结果示N-trap组结石清除率明显高于对照组,结石上移率明显低于对照组(P<0.05)。结论N-trap阻石篮能明显提升结石清除率,减少结石上移,且不增加手术时间、手术并发症及输尿管狭窄发生率。Objective To investigate the efficacy and safety of N-trap intercept stone network in the treatment of upper ureteral calculi with ureteroscopic holmium laser lithotripsy.Methods A total of 114 patients treated by ureteroscopic holmium laser lithotripsis for upper ureteral calculi in the Urology Department of Renji Hospital,Shanghai Jiaotong University School of Medicine from August 2019 to August 2022 were selected as the study objects,and were divided into the n-trap group(n=50)and the control group(n=64)according to whether n-trap lithotripsis was used.The propensity score matching method was used to further analyze and compare the stone clearance rate,stone migration rate,average operation time and surgical complications between the two groups.The incidence of postoperative ureteral stricture was compared between the two groups in the postoperative follow-up of 6-12 months.Results After the 1∶1 propensity score matching,45 cases in the N-trap group and the control group,respectively,were screened out.The stone clearance rate in the N-trap group was significantly higher than that in the control group,while the stone moving rate was significantly lower than that in the control group(P<0.05).Conclusion N-trap intercept stone network can significantly improve the stone clearance rate,reduce the upward movement of stones,and does not increase the operation time,surgical complications and the incidence of ureteral stenosis in application of ureteroscopic holmium laser lithotripsy.
