耳内镜与耳显微镜下鼓膜修补术治疗CSOM的临床疗效比较

摘　　要：目的比较耳内镜与耳显微镜下鼓膜修补术治疗慢性化脓性中耳炎(CSOM)的临床疗效。方法选取2020年1月至2022年12月在该院就诊的80例CSOM患者为研究对象,用随机数字表法分为耳内镜组、耳显微镜组,每组40例,分别对应耳内镜鼓膜修补术、耳显微镜鼓膜修补术治疗。比较两组围手术期指标、听力情况、疼痛、生活质量及不良反应发生情况。结果与耳显微镜组比较,耳内镜组手术时间、住院时间更短,术中出血量更少,疼痛程度评分更低(P<0.05)。治疗后两组气导听阈、骨导听阈、气骨导差较治疗前降低(P<0.05)。治疗后两组生活质量SF-36各维度评分高于治疗前,且耳内镜组高于耳显微镜组(P<0.05)。耳内镜组并发症发生率低于耳显微镜组(7.5%vs.12.5%),但差异无统计学意义(P>0.05)。结论耳内镜下鼓膜修补术较耳显微镜对CSOM患者疗效明显。Objective To compare the clinical efficacy of ear endoscopy and ear microscope tympanic membrane repair in the treatment of chronic suppurative otitis media(CSOM).Methods A total of 80 CSOM patients treated in this hospital from January 2020 to December 2022 were selected as the research objects,and were divided into ear endoscopy group and ear microscope group by random number table method,with 40 cases in each group,corresponding to ear endoscopy tympanic membrane repair and ear microscope tympanic membrane repair respectively.Perioperative indexes,hearing,pain,quality of life and adverse reactions were compared between the two groups.Results Compared with the ear microscope group,the operation time and hospital stay in the ear endoscope group were shorter,the intraoperative blood loss was less,and the pain degree score was lower(P<0.05).After treatment,the threshold of air conduction,bone conduction and air bone conduction were lower than those before treatment(P<0.05).The SF-36 scores of the two groups after treatment were higher than those before treatment,and the score of the ear endoscope group was higher than that of the ear microscope group(P<0.05).The incidence of complications in the ear endoscopy group was lower than that in the ear microscope group(7.5%vs.12.5%),but the difference was not statistically significant(P>0.05).Conclusion Endoscopic tympanic membrane repair is more effective than ear microscope in CSOM patients.
