凉血活血汤联合UVB光疗治疗血热性银屑病患者的临床疗效

摘　　要：【目的】探讨凉血活血汤联合窄谱中波紫外线(UVB)光疗治疗血热性银屑病患的临床疗效。【方法】选取2021年1月至2022年5月在本院诊治的80例血热性银屑病患者,按入院顺序分为对照组(尿素乳膏联合凉血活血汤治疗)和观察组(在对照组基础上加用UVB光疗治疗),每组40例,两组均治疗4周,比较两组临床疗效、银屑病面积和严重度指数(PASI)评分、角质层含水量、皮脂含量、相关因子[白细胞介素-17(IL-17)、血管内皮生长因子(VEGF)、单核细胞趋化蛋白-1(MCP-1)]水平及不良反应发生率。【结果】观察组临床疗效显著优于对照组(P<0.05)。治疗后,两组PASI评分、IL-17、VEGF、MCP-1均较治疗前降低,且观察组上述指标低于对照组(P<0.05);治疗后,两组角质层含水量、皮脂含量均较治疗前显著升高(P<0.05);且观察组角质层含水量、皮脂含量高于对照组(P<0.05)。两组患者不良反应总发生率比较,差异无统计学意义(P>0.05)。【结论】凉血活血汤联合UVB光疗治疗血热性银屑病患者,临床疗效较好,可显著改善患者的皮损程度和相关因子,且安全性良好。【Objective】To explore the clinical efficacy of Liangxue Huoxuetang combined with narrow-band ultraviolet B(UVB)phototherapy in the treatment of blood heat psoriasis patients.【Methods】A total of 80 patients with blood heat psoriasis treated in our hospital from January 2021 to May 2022 were selected and divided into a control group(treated with triamcinolone acetonide urea cream combined with Liangxue Huoxuetang)and an observation group(treated with UVB phototherapy on the basis of the control group)according to admission order,with 40 cases in each group.Both groups were treated for 4 weeks.The clinical efficacy,psoriasis area and severity index(PASI)score,stratum corneum water content,sebum content,levels of related factors[interleukin-17(IL-17),vascular endothelial growth factor(VEGF),monocyte chemoattractant protein-1(MCP-1)],and incidence of adverse reactions were compared between the two groups.【Results】The clinical efficacy of the observation group was significantly better than that of the control group(P<0.05).After treatment,the PASI scores,IL-17,VEGF,and MCP-1 in both groups decreased compared to before treatment,and the above indicators in the observation group were lower than those in the control group(P<0.05);After treatment,the water content and sebum content of the stratum corneum in both groups significantly increased compared to before treatment(P<0.05);And the water content and sebum content of the stratum corneum in observation group were higher than those in the control group(P<0.05).There was no statistically significant difference in the total incidence of adverse reactions between the two groups of patients(P>0.05).【Conclusion】The combination of Liangxue Huoxuetang and UVB phototherapy has good clinical efficacy in treating patients with blood heat psoriasis,significantly improving the degree of skin lesions and related factors,and has good safety.
