耳穴压豆联合柴芍疏肝利胆排石汤在腹腔镜下胆管切开取石术后的临床应用效果

摘　　要：目的分析腹腔镜下胆管切开取石术后采取耳穴压豆联合柴芍疏肝利胆排石汤治疗的效果。方法选取腹腔镜下胆管切开取石术后患者108例为研究对象,根据随机数字表法分为对照组与研究组,每组54例。术后肛门排气后,对照组予以熊去氧胆酸治疗,研究组予以柴芍疏肝利胆排石汤联合耳穴压豆治疗。对比两组临床效果及复发情况,治疗前后胆汁生化指标(总胆汁酸、总胆红素、非结合胆红素、钙离子)水平。结果研究组治疗总有效率96.30%显著高于对照组的83.33%,复发率3.70%低于对照组的14.81%,组间比较存在统计学差异(P<0.05)。研究组治疗后总胆汁酸、总胆红素、非结合胆红素、钙离子水平分别为(429.26±187.26)μmol/L、(218.36±19.37)μmol/L、(214.36±87.26)μmol/L、(1.13±0.11)mmol/L,低于对照组的(517.36±197.11)μmol/L、(349.36±28.36)μmol/L、(297.32±94.26)μmol/L、(1.67±0.15)mmol/L,组间比较存在统计学差异(P<0.05)。结论在腹腔镜下胆管切开取石术后予以患者耳穴压豆联合柴芍疏肝利胆排石汤治疗,能够提高手术治疗效果,改善胆汁成分,利于促进患者康复,临床可进一步推广运用。Objective To analyze the effect of auricular points plaster therapy combined with Chaishao Shugan Lidan Paishi Decoction in laparoscopic cholestrotomy.Methods A total of 108 patients who received laparoscopic cholecystectomy were selected as the study subjects,and were divided into a control group and a study group according to random number table method,each with 54 patients.After postoperative anal exhaust,the control group was treated with ursodeoxycholic acid,and the study group was treated with Chaishao Shugan Lidan Paishi Decoction and auricular points plaster therapy.The clinical effect and recurrence,and the levels of bile biochemical indexes(total bile acid,total bilirubin,unbound bilirubin,calcium ion)before and after treatment of the two groups were compared.Results The total effective rate of the study group was 96.30%,which was significantly higher than 83.33%of the control group;the recurrence rate of the study group was 3.70%,which was lower than 14.81%of the control group;there was statistical difference between groups(P<0.05).After treatment,the levels of total bile acid,total bilirubin,unbound bilirubin and calcium ion were(429.26±187.26)μmol/L,(218.36±19.37)μmol/L,(214.36±87.26)μmol/L and(1.13±0.11)mmol/L in the study group,which were lower than(517.36±197.11)μmol/L,(349.36±28.36)μmol/L,(297.32±94.26)μmol/L and(1.67±0.15)mmol/L in the control group.There were statistical differences between groups(P<0.05).Conclusion Auricular points plaster therapy combined with Chaishao Shugan Lidan Paishi Decoction for patients after laparoscopic cholecystectomy can improve the effect of surgical treatment,improve the composition of bile,and promote the rehabilitation of patients,which can be further popularized and applied in clinic.
