鞣酸苦参碱片联合抗病毒疗法对艾滋病相关腹泻的疗效观察

摘　　要：目的 观察鞣酸苦参碱片对艾滋病伴腹泻患者的辅助治疗效果及对免疫功能的影响。方法 通过比较试验组和对照组的症状计分值来判断鞣酸苦参碱片的辅助疗效;通过比较两组患者治疗前后CD4~+ T细胞、白细胞的变化,观察鞣酸苦参碱片对患者免疫功能的影响。结果 症状轻重程度分级计分结果显示,从大便次数来看,鞣酸苦参碱片辅助治疗组和抗病毒+常规西药组(如奥曲肽、易蒙停、地芬诺酯等)治疗艾滋病伴腹泻患者的有效率分别为83.33%、76.67%;从大便性质改变来看,两组治疗有效率分别为90%、83.33%、两组患者腹痛程度治疗的有效率均为43.33%;从大便次数改善方面来看,经鞣酸苦参碱片辅助治疗后的分值下降程度显著高于抗病毒+常规西药组;从大便性质和腹痛程度的改善来看,两种治疗方法没有显著性差异。免疫学层面分析,无论是鞣酸苦参碱片辅助治疗,还是抗病毒+常规西药治疗,CD4~+T细胞和白细胞的数量都有所提升,其中,CD4~+T细胞升高显著,差别有统计学意义,白细胞升高不显著,差别无统计学意义。结论 经鞣酸苦参碱片辅助治疗艾滋病伴腹泻患者在症状改善方面效果优于抗病毒+常规西药治疗,可以作为艾滋病抗病毒治疗过程中的一种辅助手段。Objective To observe the adjuvant treatment effect of Sophora Alkaloids Tannate tablets on AIDS patients with diarrhea and its influence on immune function. Methods The auxiliary efficacy of Sophora Alkaloids Tannate tablets was evaluated by comparing the symptom scores and the daily frequency of diarrhea between the experimental group and the control group. The changes of CD4~+ T cells and white blood cells before and after the treatment were compared between the two groups to observe the effect of sophora alkaloids tannate tablets on the immune function of patients before and after the treatment. Results Symptom severity grading score statistics showed that from the point of stool frequency, the effective rate of adjuvant treatment with sophora alkaloids tannate tablets and simple conventional western medicine(such as octreotide, Imodil, phenytopidine, and etc.) in the treatment of AIDS patients with diarrhea was 83.33% and 76.67%, respectively. According to fecal property, the effective rates of the two groups were 90% and 83.33% respectively. The effective rate of abdominal pain treatment in both groups was 43.33%. From the improvement of stool frequency, the reduction degree of the score after the adjuvant treatment with sophora alkaloids tannate tablets was significantly greater than that of the conventional western medicine group. There were no significant differences between the two treatments in terms of stool quality and abdominal pain. Immunological analysis showed that the number of CD4~+T lymphocytes and white blood cells increased slightly both with the adjuvant treatment of sophora alkaloids tannate tablets and with conventional western medicine. Among them, CD4~+ T cells were significantly increased, and the difference was statistically significant, The elevation of white blood cells was not significant, and the difference was not statistically significant.Conclusion The effect of Sophora alkaloids tannate tablets is better than that of conventional western medicine in the treatment of AIDS patien
