针刀联合中药熏洗治疗胫骨平台骨折术后膝关节僵硬临床观察

摘　　要：目的探讨针刀联合中药熏洗治疗胫骨平台骨折术后膝关节僵硬的临床效果。方法选择上饶市万年县中医院2018年6月—2021年12月收治的60例胫骨平台骨折术后膝关节僵硬患者,随机分为对照组和观察组,各30例。对照组单纯使用手法放松,观察组在对照组基础上辅以小针刀和中药熏洗治疗,疗程均为30 d。观察两组并发症发生情况及治疗前后膝关节被动屈曲最大活动角度、视觉模拟量表(VAS)评分、美国特种外科医院(HSS)膝关节评分。结果两组均未发生骨折、感染、烫伤等并发症。治疗后,两组膝关节屈曲角度、HSS评分均显著提高,且观察组高于对照组(P<0.05);两组VAS评分均明显降低,且观察组低于对照组(P<0.05)。结论针刀联合中药熏洗治疗胫骨平台骨折术后膝关节僵硬,可有效改善患者膝关节活动度,减轻疼痛,提高关节总体功能,是有效的干预措施,值得临床推广。Objective To investigate the clinical effect of needle knife combined with Chinese medicine fumigation on knee stiffness after tibial plateau fracture.Methods A total of 60 patients with knee joint stiffness after tibial plateau fracture operation in Wannian Hospital of Traditional Chinese Medicine from June 2018 to December 2021 were selected as research objects and randomly divided into control group and observation group on average.The control group only used manual relaxation.On the basis of control group,observation group was supplemented with needle knife combined with Chinese medicine fumigation.The course of treatment was 30 days.The clinical efficacy and complications of the two groups were evaluated,and the maximum passive knee flexion Angle,visual analog scale(VAS)score and HSS knee score before and after treatment were compared.Results No complications such as fracture,infection and scald occurred in the two groups.Knee flexion Angle and HSS score in 2 groups were significantly improved after treatment,and the observation group was significantly higher than that in control group(P<0.05).VAS scores in both groups were significantly decreased after treatment,and the observation group was lower than the control group(P<0.05).Conclusion Needle knife combined with Chinese medicine fumigation therapy in the treatment of knee stiffness after tibial plateau fracture can effectively improve the range of knee motion,reduce pain,improve the overall joint function,which is an effective intervention measure and is worthy of clinical promotion.
