香芍颗粒治疗子宫内膜异位症术后使用GnRH-a致类绝经症状临床观察

摘　　要：目的探讨香芍颗粒治疗子宫内膜异位症术后使用促性腺激素释放激素激动剂(GnRH-a)致类绝经症状的临床效果。方法选择2021年9月—2022年9月在江西省妇幼保健院妇科就诊的Ⅲ~Ⅳ期子宫内膜异位症患者84例,根据随机数字表法分为两组,每组42例。观察组术后给予香芍颗粒联合GnRH-a治疗,对照组术后仅给予GnRH-a治疗。比较3个月后两组治疗前后的Kupperman评分、性激素水平及类绝经症状发生情况。结果治疗前,两组患者的Kupperman评分及性激素水平比较,差异无统计学意义(P>0.05)。治疗后,观察组Kupperman评分低于对照组,促卵泡生长激素(FSH)、促黄体生成素(LH)水平低于治疗组,雌二醇(E2)水平高于对照组,差异均有统计学意义(P<0.05)。观察组出现类绝经症状的例数明显少于对照组。结论在子宫内膜异位症术后患者的巩固治疗中加用香芍颗粒可以缓解类绝经症状,改善卵巢功能,具有临床应用价值。Objective To investigate the clinical efficacy of Xiangshao granule in treating menopausal symptoms caused by GnRH-a in patients with endometriosis after operation.Methods From September 2021 to September 2022,84 patients with stage III-IV endometriosis who were diagnosed by surgery and pathology in Jiangxi Maternal and Child Health Hospital were selected and divided into two groups according to the random number table,with 42 patients in each group.The observation group was treated with Xiangshao granules and GnRH-a for 3 months after operation,while the control group was only treated with GnRH-a for 3 months after operation.Kupperman score and sex hormone level were compared between the two groups before and after treatment,and adverse reactions were recorded.Results Before treatment,there was no significant difference in Kupperman score and sex hormone level between 2 groups(P>0.05).After treatment,the Kupperman score of observation group was lower than that of control group,the levels of follicle stimulating hormone(FSH)and luteinizing hormone(LH)were lower than those of treatment group,and the level of estradiol(E2)was higher than that of control group,with statistical significance(P<0.05).The number of cases of menopausal symptoms in the observation group was significantly less than that in the control group.Conclusion Xiangshao granules can alleviate menopausal symptoms and improve ovarian function in the consolidation treatment of endometriosis after operation,which has clinical application value.
