超声引导下细针抽吸冲洗联合真空拔罐对脓肿型浆细胞性乳腺炎的治疗效果研究

摘　　要：目的观察超声引导下细针抽吸冲洗联合真空拔罐治疗脓肿型浆细胞性乳腺炎(PCM)的临床疗效。方法回顾性分析于我院就诊的60例脓肿型PCM患者,按照患者的治疗方式进行分组,穿刺抽液组30例,在超声引导下细针穿刺抽液联合局部注射曲安奈德;真空拔罐组30例,在穿刺抽液组治疗的基础上行真空拔罐器负压抽吸术。分析比较两组的治疗效果、疼痛情况等。结果两组治疗总有效率、复发率、治疗前疼痛视觉模拟评分(VAS)比较差异无统计学意义(P>0.05)。真空拔罐组病灶缩小一半所需时间、治疗时间及病程均短于穿刺抽液组,且真空拔罐组治疗后病灶最大径显著小于穿刺抽液组,两组比较差异有统计学意义(P<0.05)。两组治疗2周后VAS评分均较治疗前改善,且真空拔罐组治疗后VAS评分低于穿刺抽液组,其差异均有统计学意义(P<0.05)。结论两组疗效差异无统计学意义,超声引导下细针抽吸冲洗联合真空拔罐器负压抽吸治疗能有效缩小肿块范围、缩短治疗时间、缓解疼痛症状。Objective To observe the clinical efficacy of treating purulent plasma cell mastitis(PCM)with ultrasound-guided fine needle aspiration irrigation combined with vacuum cupping.Methods A retrospective analysis was conducted on 60 patients diagnosed with purulent PCM in our Hospital.The patients were divided into two groups based on their treatment modalities.The fine needle aspiration group consisted of 30 patients who underwent ultrasound-guided fine needle aspiration combined with local injection of triamcinolone ace-tonide;the vacuum cupping group comprised 30 patients who received vacuum cupping suction therapy in addi-tion to the treatment provided to the aspiration group.The therapeutic outcomes,pain levels,and other relevant data were analyzed and compared between the two groups.Results There were no statistically significant differ-ences between the two groups in terms of overall treatment effectiveness,recurrence rates,and pain Visual Ana-logue Scale(VAS)scores before treatment(P>0.05).The time required for lesion reduction by half,the dura-tion of treatment,and the course of the disease were all shorter in the vacuum cupping group compared to the as-piration group,and the maximum diameter of the lesion after treatment in the vacuum cupping group was signif-icantly smaller than that in the aspiration group,with statistically significant differences(P<0.05).Both groups showed improvement in pain VAS scores two weeks after treatment,with the vacuum cupping group ex-hibiting lower post-treatment VAS scores than the aspiration group,indicating statistically significant differ-ences(P<0.05).Conclusion There were no significant differences in the efficacy between the two groups.The combination of ultrasound-guided fine needle aspiration irrigation with vacuum cupping suction therapy can effectively reduce the size of the lesion,shorten the treatment duration,and alleviate pain symptoms.
