补肾助孕方对复发性流产患者子宫内膜容受性及妊娠结局影响的临床研究

摘　　要：目的观察补肾助孕方对复发性流产(recurrent spontaneous abortion,RSA)患者子宫内膜容受性及妊娠结局的影响。方法纳入2019年10月—2021年3月就诊于广安门医院妇科的符合复发性流产诊断的患者共100例,按照随机数字表法分成对照组50例和治疗组50例。观测指标包括治疗前后中医证候评分、月经情况评分、子宫内膜容受性相关指标和妊娠结局等。结果对照组有3例患者失访,治疗组有2例患者治疗期间未按规定试验方案服药剔除,最终95例患者完成本次研究,其中对照组47例,治疗组48例。治疗3个月经周期后,治疗组总有效率、月经情况改善、临床妊娠率均优于对照组,中医证候评分降低,差异具有统计学意义(P<0.05)。治疗后治疗组的子宫内膜厚度、血流分布和Salle评分均得到明显优化,血流阻力指数(resistance index,RI)显著降低,差异具有统计学意义(P<0.05),在血流灌注指数(perfusion index,PI)方面差异无统计学意义(P>0.05)。随访后发现治疗组临床妊娠率显著升高,妊娠失败例数低于对照组,具差异有统计学意义(P<0.05)。两组患者不良反应发生率比较差异无统计学意义(P>0.05)。结论研究表明补肾助孕方可改善肾虚中医证候,提高RSA患者临床妊娠率。补肾助孕方具有明显促进子宫内膜生长、增加子宫内膜血流灌注、降低RI和PI的作用,验证了补肾助孕方孕前干预可提高子宫内膜容受性治疗RSA。Objective To observe the Effect of Bushen Zhuyun Decoction(补肾助孕方)on endometrial receptivity and pregnancy outcome in patients with recurrent spontaneous abortion.Methods A total of 100 patients with recurrent spontaneous abortion treated in the hospital from October 2019 to March 2021 were selected and divided into control group(50 cases)and test group(50 cases)according to random number table.The examination indexes of this study included TCM syndrome scores,menstrual condition score,endometrial receptivity related indexes and pregnancy outcome before and after treatment.Results During the study,3 patients in the control group were lost to follow-up,and 2 patients in the test group did not take medicine according to the prescribed test protocol during the treatment period.Finally,95 patients completed the study,including 47 patients in the control group and 48 patients in the test group.After treatment,the total effective rate,menstrual condition improvement and clinical pregnancy rate of the test group were better than those of the control group,and the TCM syndrome scores were decreased,with a statistically significant difference(P<0.05).After treatment,endometrial thickness,morphology and blood flow distribution of the test group were significantly optimized,the resistance index(RI)was significantly reduced with statistical significance(P<0.05)and there was no significant difference in perfusion index(PI)(P>0.05).After follow-up,it was found that the clinical pregnancy rate was significantly higher and the number of pregnancy failures was lower in the test group than thsoe in the control group(P<0.05).There was no significant difference in the incidence of adverse effects between the two groups(P>0.05).Conclusion The study suggested that Bushen Zhuyun Decoction could improve TCM syndromes and improve clinical pregnancy rates in RSA patients.Therefore,the preconception intervention by Bushen Zhuyun Decoction could obviously promote endometrium growth,increase endometrial blood flow perfusion and redu
