基于“宫络学说”探讨“气化理论”在防治宫腔粘连所致反复移植失败中的应用

摘　　要：反复移植失败(repeated/recurrent implantation failure,RIF)是多次胚胎移植后未获得临床妊娠者,随着辅助生殖技术(assisted reproductive technology,ART)的快速发展,其发病率日益增高,成为辅助生殖领域面临的一大挑战。临床发现RIF与宫腔粘连(intrauterine adhesions,IUA)密切相关,对于该类患者,治疗主要采取宫腔镜宫腔粘连分离术(transcervical resection of adhesions,TCRA),但术后复粘率高,着床率低等问题尚待解决。导师姜丽娟教授在“宫络学说”及“气化理论”思想指导下,分析其与RIF及IUA相关性,以“和畅子宫络脉法”为主要治疗原则,将“气化理论”贯穿于临床诊治全过程,对受损子宫内膜的修复具有良好疗效,能够显著提升子宫内膜容受性,提高临床妊娠率。Recurrent implantation failure(RIF)is no clinical pregnancy after multiple multiple embryo transplantation.With the rapid development of the assisted reproductive technology(ART),its incidence is increasing,which has become a major challenge in the field of assisted reproduction.It is noticed that RIF is closely related to intrauterine adhesions(IUA),and the transcervical resection of adhesions(TCRA)is the main treatment for the patients.However,the problems such as high re-adhesion rate and low implantation rate remain to be solved.Under the thought of“Qi transformation”and“uterine collateral theory”,the tutor professor JIANG Lijuan analyzed the relationship of RIF,IUA and these theories.Guided by the method of harmonizing uterine collateral,professor JIANG applied the theory of“Qi transformation”throughout the whole process of clinical diagnosis and treatment,gained a good effect on the repair of damaged endometrium,significantly improved the endometrial receptivity and the clinical pregnancy rate.
