邓氏痛风方内服联合平安散外敷治疗急性痛风性关节炎疗效观察

摘　　要：目的探讨急性痛风性关节炎经邓氏痛风方内服联合平安散外敷治疗的临床效果。方法选择该院收治2020年6月—2022年6月的急性痛风性关节炎患者100例,将其划分为观察组、对照组,每组均为50例。给予对照组患者西药常规口服治疗,在对照组治疗方案的基础上,联合给予观察组患者中医邓氏痛风方内服联合平安散外敷治疗,均给予两组患者1周的治疗时间。将观察组、对照组患者治疗后临床疗效、治疗前后主要症状积分、Likert指数评分、血清白细胞介素-1β(IL-1β)、红细胞沉降率(ESR)、环氧化酶-2(COX-2)、基质金属蛋白9(MMP9)水平、实验室指标,治疗期间安全性进行统一的对比与分析。结果治疗后,观察组患者的临床总有效率98.00%(49/50)较对照组的86.00%(43/50)相比,更高;治疗后,两组患者关节触痛度、关节疼痛度、关节肿胀度、关节活动受限症状各项评分、Likert指数评分、IL-1β、ESR、COX-2、MMP9、血清黄嘌呤氧化酶(XOD)、血尿酸(BUA)、肌酐(Cr)水平与治疗前相比较,均呈现降低趋势,其中较对照组比,观察组处于较低水平(P<0.05),提示其数据经计算有显著性差异。结论邓氏痛风方内服联合平安散外敷治疗急性痛风性关节炎效果显著,可改善患者证候及关节功能,同时有助于减轻机体尿酸水平及炎性损伤,整体安全性高。Objective To investigate the clinical effect of Deng′s Tongfeng Prescription(邓氏痛风方)combined with external application of Ping′an Powder(平安散)in the treatment of acute gouty arthritis.Methods A hundred patients with acute gouty arthritis admitted to the hospital from June 2020 to June 2022 were divided into the observation group and the control group,with 50 cases in each group.The control group was given conventional oral treatment of Western medicine.On the basis of the treatment plan of the control group,the observation group was combined with internal administration of Deng′s Tongfeng Prescription and external application of Ping′an Powder,and the treatment time of 1 week was given to both groups.Clinical efficacy,main symptom scores,Likert index score,serum levels of interleukin-1β(IL-1β),red blood cell sedimentation rate(ESR),cyclooxygenase-2(COX-2)and matrix metalloproteinase-9(MMP9)and laboratory indexes before and after treatment and the safety during treatment were uniformly compared and analyzed in the observation group and the control group.Results After treatment,the total effective rate of the observation group was 98.00%(49/50),which was higher than that of control group(86.00%,43/50).After treatment,the levels of joint tenderness,joint pain,joint swelling,joint restriction symptom scores,Likert index scores,serum levels of IL-1β,ESR,COX-2,MMP9,xanthine oxidase(XOD),blood uric acid(BUA)and creatinine(Cr)in two groups showed a decreasing trend compared with those before treatment,and the observation group had a lower level than the control group(P<0.05),indicating a significant difference in the calculated data.Conclusion Internal administration of Deng′s Tongfeng Prescription combined with external application of Ping′an powder has significant effect in the treatment of acute gouty arthritis,which can improve the syndrome and joint function of patients,and help to reduce the level of uric acid and inflammatory damage of the body,with high overall safety.
