黄芪建中汤联合艾灸对腹泻型肠易激综合征患者临床症状、炎症反应以及胃肠激素的影响

摘　　要：目的探究黄芪建中汤联合艾灸对腹泻型肠易激综合征(diarrheal irritable bowel syndrome,IBS-D)患者的临床症状、炎症反应以及胃肠激素的影响。方法选取2020年1月—2022年10月期间该院接收的IBS-D患者80例,采用随机数表法分为观察组(40例)和对照组(40例)。所有患者均行常规西药治疗,对照组给予艾灸,观察组在其基础上联合黄芪建中汤治疗。对比两组患者的治疗疗效,治疗前后炎症指标[可溶性白细胞介素2受体(soluble interleukin-2 receptor,SIL-2R)、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)、白细胞介素-1β(interleukin-1β,IL-1β)]以及胃肠激素指标[胃泌素(gastrin,GAS)、胃动素(motilin,MTL)、生长抑素(somatostatin,SS)]血清浓度变化。同时采用IBS生活质量问卷(IBS quality of life questionnaire,IBS-QOL)、汉密顿抑郁量表(Hamilton depression scale,HAMD)评估所有患者心理状态以及生活质量变化。结果观察组患者的治疗总有效率高于对照组(P<0.05)。治疗后,观察组血清SIL-2R、L-1β、TNF-α以及SS均低于对照组(P<0.05),MTL、GAS浓度高于对照组(P<0.05)。治疗后,观察组HAMD评分低于对照组(P<0.05),IBS-QPL评分高于对照组(P<0.05)。结论黄芪建中汤联合艾灸对可以有效改善IBS-D患者的临床症状,改善其炎症水平以及胃肠功能,缓解其不良情绪、改善患者的生活质量。Objective To explore the effects of Huangqi Jianzhong Decoction(黄芪建中汤)combined with moxibustion on clinical symptoms,inflammatory response and gastrointestinal hormones in patients with diarrheal irritable bowel syndrome(IBS-D).Methods A total of 80 IBS-D patients admitted to the hospital between January 2020 and October 2022 were selected and divided into observation group(40 cases)and control group(40 cases)by random number table method.All patients were treated with conventional western medicine.The control group was given moxibustion,and the observation group was treated with Huangqi Jianzhong Decoction.The therapeutic efficacy of the two groups was compared.Pre-and post-treatment inflammatory markers[soluble interleukin-2 receptor(SIL2R),tumor necrosis factor-α(TNF-α),interleukin-1β(IL-1β)]and serum concentrations of gastrointestinal hormone indexes[gastrin(GAS),motilin(MTL),somatostatin(SS)].At the same time,the IBS quality of life questionnaire(IBS-QOL)and Hamilton depression scale(HAMD)were used to evaluate the mental state and changes in quality of life of all patients.Results The total effective rate of the observation group was higher than that of the control group(P<0.05).After treatment,the levels of serum SIL-2R,L-1β,TNF-αand SS in the observation group were lower than those in the control group(P<0.05),while the concentrations of MTL and GAS were higher than those in the control group(P<0.05).After treatment,HAMD score of the observation group was lower than that of the control group(P<0.05)and the IBS-QPL score was higher than that of the control group(P<0.05).Conclusion Huangqi Jianzhong Decoction combined with moxibustion can effectively improve the clinical symptoms of IBS-D patients,improve the level of inflammation and gastrointestinal function,relieve the adverse mood and improve the quality of life of patients.
