生肌解毒方外用联合富血小板血浆技术在烧伤后慢性创面中的应用

摘　　要：目的探究生肌解毒方外用联合富血小板血浆(platelet rich plasma,PRP)技术在烧伤后慢性创面的应用情况。方法选取2020年5月—2021年5月该院收治的104例烧伤慢性创面患者为研究对象,随机分为对照组和观察组,各52例,对照组给予PRP技术治疗,观察组在对照组基础上外用生肌解毒方。比较两组创面愈合率和愈合时间、创面量化积分和视觉模拟评分(visual analog score,VAS)、炎性因子[C反应蛋白(C-reactive protein,CRP)、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)、白细胞计数(white blood cell count,WBC)]、血管内皮细胞生长因子(vascular endothelial growth factor,VEGF)和基质金属蛋白酶-9(matrix metalloproteinase-9,MMP-9)及不良反应。结果相较于对照组,观察组第1周创面愈合率略高(P>0.05),第3、5周创面愈合率明显较高(P<0.05),创面愈合时间明显较短(P<0.05);治疗后相较于对照组,观察组创面量化积分和VAS均明显较低(P<0.05),CRP、TNF-ɑ、WBC水平均明显较低(P<0.05),VEGF明显较高(P<0.05),MMP-9明显较低(P<0.05);两组不良反应总发生率差异无统计学意义(P>0.05)。结论生肌解毒方外用联合PRP技术可促进烧伤后慢性创面愈合,缓解创面症状和疼痛,减轻炎症反应,调节VEGF和MMP-9水平,安全性较好。Objective To investigate the application of Shengji Jiedu Recipe(生肌解毒方)Prescription combined with platelet rich plasma(PRP)in chronic burn wounds.Methods A hundred and four104 patients with chronic burn wounds admitted to ourthe hospital from May 2020 to May 2021 were selected as the research objects,and randomly divided into the control group and the observation group,with 52 patients in each group.The control group was treated with PRP technology,and the observation group was treated with Shengji Jiedu RecipeDecoction on the basis of the control group.The wound healing rate and time,wound quantitative score and visual analog score(VAS),inflammatory factors[C-reactive protein(CRP),tumor necrosis factor-α(TNF-α),white blood cell count(WBC)],vascular endothelial growth factor(VEGF),matrix metalloproteinase-9(MMP-9)and adverse reactions were compared between the two groups.Results Compared with those of the control group,the wound healing rate of the observation group was slightly higher in the first week(P>0.05),the wound healing rate was significantly higher in the third and fifth weeks(P<0.05),and the wound healing time was significantly shorter(P<0.05);.After treatment,compared with those of the control group,the quantitative score of wound surface and VAS in the observation group were significantly lower(P<0.05).The levels of CRP,TNF-α、and WBC level wasere significantly lower(P<0.05),the level of VEGF was significantly higher(P<0.05),and the level of MMP-9 was significantly lower(P<0.05);.There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion The external application of Shengji Jiedu RecipePrescription combined with PRP technology can promote the chronic wound healing after burns,relieve wound symptoms and pain,reduce the inflammatory reaction,and regulate the levels of VEGF and MMP-9,with good safety.
