血浆D二聚体联合降钙素原白细胞介素6检测对孕产妇B族链球菌感染的诊断价值

摘　　要：目的评估血浆D二聚体、降钙素原(PCT)和白细胞介素6(IL6)联合检测在诊断妊娠晚期孕妇B族链球菌(GBS)感染中的价值。方法于2021年9月至2022年9月期间收集GBS阳性的妊娠晚期孕妇42例作为试验组,以及同期GBS阴性孕妇30例作为对照组。所有参与者均接受了血浆D二聚体、PCT和IL6水平的检测。结果试验组和对照组在年龄和孕周方面差异无统计学意义(P>0.05)。试验组的血浆D二聚体(0.89±0.15)mg/L、PCT(0.085±0.010)ng/ml、IL6(18.8±2.1)pg/ml水平显著高于对照组[(0.54±0.11)mg/L,(0.039±0.005)ng/ml,(9.7±0.8)pg/ml,P<0.01]。血浆D二聚体、PCT和IL6检测的灵敏度分别为76.2%、80.9%和83.3%,特异度分别为36.7%、40.0%和36.7%。联合检测GBS感染的诊断灵敏度和特异度阳性预测值分别为92.9%、56.7%和75.0%。结论血浆D二聚体、PCT和IL6的升高可能提示GBS感染的存在。3项联合检测可以提高诊断准确性。Objective To evaluate the value of combined plasma Ddimer,PCT(procalcitonin)and interleukin6(IL6)tests in the diagnosis of Group B Streptococcus(GBS)infection in pregnant women in late pregnancy.Methods Fortytwo GBSpositive pregnant women in late pregnancy were collected as an experimental group and 30 GBSnegative pregnant women as a control group from September 2021 to September 2022.All participants were tested for plasma Ddimer,PCT and IL6 levels.Results The differences between the experimental and control groups in terms of age and gestational weeks were not significant(P>0.05).Plasma Ddimer(0.89±0.15)mg/L,PCT(0.085±0.010)ng/ml,and IL6(18.8±2.1)pg/ml levels were significantly higher in the experimental group than those in the control group[Ddimer:(0.54±0.11)mg/L,PCT:(0.039±0.005)ng/ml,IL6:(9.7±0.8)pg/ml,all P<0.01].The sensitivity of plasma Ddimer,PCT and IL6 assays was 76.2%,80.9%and 83.3%,respectively,and the specificity was 36.7%,40.0%and 36.7%,respectively.The combined assay showed higher comprehensive performance and improved diagnostic sensitivity,specificity and the positive value for GBS infection was 92.9%、56.7%vs 75.0%.Conclusion This study demonstrated that the combined assay of plasma Ddimer,PCT and IL6 showed significant diagnostic value in the diagnosis of maternal GBS infection with statistical significance.The combined use of these biochemical indicators may improve diagnostic accuracy,provide more precise clinical management and intervention for pregnant women,and help optimize health management strategies for pregnant women and newborns.
