摘  要:目的探讨N乙酰半胱氨酸联合噻托溴铵对慢性阻塞性肺疾病急性加重期(AECOPD)患者血清超敏C反应蛋白(hsCRP)、人中性粒细胞肽13(HNP13)、五聚素3(PTX3)及肺功能的影响。方法本研究为前瞻性研究,102例患者均来源于济源市中医院,选取时间为2019年8月至2022年8月,按随机数字表法分为对照组(51例,噻托溴铵吸入治疗)和研究组(51例,在噻托溴铵吸入治疗的基础上加用N乙酰半胱氨酸治疗)。2组患者均进行为期14 d的治疗。将2组患者治疗效果,治疗前后肺功能指标的变化、血清hsCRP、HNP13、PTX3及氧化应激指标水平进行对比。结果研究组与对照组患者的临床总有效率分别为88.2%(45/51)、72.6%(37/51),研究组比对照组高;治疗后2组患者肺功能指标、超氧化物歧化酶(SOD)均较治疗前上升,且研究组患者上述指标比对照组高,而治疗后2组患者血清hsCRP、HNP13、PTX3及丙二醛(MDA)、一氧化氮(NO)指标水平均较治疗前下降,且研究组患者均低于对照组(P<0.05)。结论N乙酰半胱氨酸联合噻托溴铵治疗急性加重期COPD患者,可提升肺功能指标水平,对氧化应激反应予以调节,减轻炎症反应,临床疗效确切。Objective To investigate the effects of Nacetylcysteine plus tiotropium on serum hypersensitivityCreactive protein(hsCRP),human neutrophil peptide 13(HNP 13),pentamerin 3(PTX3)and lung function in patients with acute exacerbation of chronic obstructive pulmonary disease(COPD).Methods This study is a prospective study with 102 patients selected from Jiyuan Hospital of Traditional Chinese Medicine from August 2019 to August 2022 and divided into control group(51 patients,tiotropium inhalation therapy)and study group(51 patients treated with Nacetylcysteine plus tiotropium inhalation therapy).Both groups were treated for 14 days.The treatment effect of patients,the change of pulmonary function index before and after treatmwent,serum hsCRP,HNP 13,PTX3 and oxidative stress index levels were compared between the two groups.Results The total clinical response rate of the study group and control group was 88.2%(45/51)and 72.6%(37/51)respectively,higher than those of the control group;the lung function index,superoxide dismutase(SOD)were higher than those before treatment,and the serum hsCRP,HNP 13,PTX3,and malondialdehyde(MDA)and nitric oxide(NO)were lower than those before treatment,and the treatment index was lower than that of the control group(all P<0.05).Conclusion Nacetylcysteine and tiotropium in COPD patients with acute exacerbation can improve the level of lung function index,regulate oxidative stress response,reduce inflammation,and have definite clinical effect.
