一种新型宫颈液基细胞标本制作细胞蜡块的方法

摘　　要：目的寻找一种减少细胞损失,提高诊断准确率的宫颈液基标本细胞蜡块制作方法。方法收集液基薄层细胞学检查(TCT)诊断为高级别鳞状上皮内瘤变(HSIL)、低级别鳞状上皮内瘤变(LSIL)的妇科宫颈液基细胞标本各30例,用新型的方法制作细胞蜡块,行苏木精-伊红(HE)染色和P16、Ki67免疫组织化学染色,对比与TCT的符合率,再将此方法运用于非典型鳞状细胞-不能除外高级别鳞状上皮内瘤变(ASCH)。结果高级别鳞状上皮内瘤变细胞蜡块与TCT诊断符合率为100%(30/30),低级别鳞状上皮内病变细胞蜡块与TCT诊断符合率为93.3%(28/30),差异无统计学意义(P>0.05)。结论此种细胞蜡块制作方法制作的细胞蜡块与TCT诊断结果符合率高,可用于ASCH的进一步诊断。Objective To explore a method for making cell paraffin blocks from liquidbased cervical cytology specimens in order to reduce cell loss and improve diagnostic accuracy.Methods Thirty cases each of gynecological cervical liquidbased cytology specimens diagnosed by Thinlayer cervical cytology test(TCT)as highgrade squamous intraepithelial lesion(HSIL)and lowgrade squamous intraepithelial lesion(LSIL)were collected.A novel method was used to prepare cell paraffin blocks,and HE staining along with P16 and Ki67 immunohistochemical staining were performed.The consistency rate with TCT was compared,and this method was further applied to atypical squamous cellscannot exclude highgrade squamous intraepithelial lesions(ASCH).Results The consistency rate between the highgrade squamous intraepithelial lesion cell paraffin blocks and TCT diagnosis was 100%,and the consistency rate between the lowgrade squamous intraepithelial lesion cell paraffin blocks and TCT diagnosis was 93.3%,with no statistical significance(P>0.05).Conclusion The cell paraffin block preparation method proposed here has a high consistency rate with TCT diagnostic results and can be used for further diagnosis of atypical squamous cellscannot exclude highgrade squamous intraepithelial lesions.
