隔盐龟板灸联合针刺治疗肾阳虚型非特异性腰痛临床研究

摘　　要：目的 探究隔盐龟板灸联合针刺治疗肾阳虚型非特异性腰痛(nonspecific low back pain, NSLBP)的临床疗效。方法 选取2023年4月—2024年3月本院收治的60例肾阳虚型NSLBP患者,按照随机数字表法分为试验组和对照组,每组30例。对照组予以针刺治疗,试验组在对照组治疗基础上加用隔盐龟板灸,2组均治疗4周。比较2组临床疗效及治疗前后中医证候积分、腰部疼痛评分、腰部功能评分、炎性介质水平、疼痛介质水平。结果 治疗后,试验组患者血清白细胞介素-6(interleukin-6,IL-6)、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)、5-羟色胺(5-hydroxytryptamine, 5-HT)、前列腺素E_(2)(prostaglandin E_(2),PGE_(2))水平显著低于对照组(P<0.05)。试验组患者中医证候积分、视觉模拟评分法(visual analogue scale, VAS)评分显著低于对照组(P<0.05);日本骨科协会评估治疗(Japan orthopedic association, JOA)评分显著高于对照组(P<0.05)。试验组总有效率显著高于对照组(P<0.05)。结论 隔盐龟板灸联合针刺治疗肾阳虚型NSLBP患者可缓解腰部疼痛,改善腰部功能,抑制炎性介质和疼痛介质释放,提高临床疗效。Objective To investigate the clinical efficacy of salt separated turtle plate moxibustion combined with acupuncture in the treatment of nonspecific low back pain(NSLBP)with kidney yang deficiency type.Methods Sixty cases of NSLBP patients with kidney yang deficiency type from April 2023 to March 2024 in our hospital were selected and divided into the test group and the control group according to the random number table method,with 30 cases in each group.The control group was treated with acupuncture,while the test group was treated with salt separated turtle plate moxibustion on the basis of treament in the control group.Both groups were treated for four weeks.The clinical effect,traditional Chinese medicine(TCM)syndrome score,lumbar pain score,lumbar function score,inflammatory mediator level and pain mediator level were compared between the two groups before and after treatment.Results After treatment,the serum levels of interleukin-6(IL-6),tumor necrosis factor-α(TNF-α),5-hydroxytryptamine(5-HT),and prostaglandin E_(2)(PGE_(2))in the test group were significantly lower than those in the control group(P<0.05).The scores of TCM syndrome and visual analogue scale(VAS)in the test group were significantly lower than those in the control group(P<0.05),and the scores of the Japan orthopedic association(JOA)were significantly higher than those in the control group(P<0.05).The total effective rate of the test group was significantly higher than that of the control group(P<0.05).Conclusion The combination of salt separated turtle plate moxibustion and acupuncture in the treatment of NSLBP patients with kidney yang deficiency type could alleviate lower back pain,improve lower back function,inhibit the release of inflammatory and pain mediators,and improve clinical efficacy.
