态靶辨证在肝郁脾虚型桥本氏甲状腺炎中的运用

摘　　要：桥本氏甲状腺炎是临床常见的自身免疫性疾病,中医病机责之于气滞、血瘀、痰凝,病位在肝脾肾,可涉及于心,病性虚实夹杂、本虚标实;导师朴春丽教授师从仝小林院士,将仝小林院士“态靶医学”的辨证组方体系运用于桥本氏甲状腺炎,以柴归类方疏肝解郁,抑木扶土,调病之“态”;针对本病TPOAb及TgAb两项抗体滴度、肝酶升高之“标靶”和情志不舒、瘿肿、突眼等“症靶”,选择相应的靶药治疗,临床疗效显著。Hashimoto's thyroiditis(HT)is a common autoimmune disease in clinical practice,whose pathogeneses of traditional Chinese medicine are qi stagnation,blood stasis,phlegm coagulation,whose affected areas are located in the liver,spleen and kidney,with the heart maybe involved as well,and whose characteristics include intermingled deficiency and excess,deficient root and excessive manifestations.Professor Piao Chunli,studying under Academician Tong Xiaolin,applies the syndrome differentiation and prescription system of"state-target medicine"developed by Academician Tong Xiaolin to the diagnosis and treatment of Hashimoto's thyroiditis.She uses classified prescriptions of Chinese thorowax root and Chinese angelica to achieve the goals of dispersing stagnated liver qi for relieving qi stagnation,suppressing wood and supporting earth,to regulate the"states"of the disease.According to the"biochemical indicator targets"of TPOAb and TgAb elevated titers and liver enzymes,and the"symptom targets"such as emotional discomfort,gall,and exophthalmos in this disease,she chooses the corresponding targeted drugs,realizing a remarkable clinical effect.
