摘 要:目的:通过前瞻性、自身半脸对照研究,比较侵入性微针射频与超脉冲二氧化碳(CO_(2))点阵激光治疗面部萎缩性痤疮瘢痕的疗效与安全性。方法:选取26例面部萎缩性痤疮瘢痕患者,面部两侧随机分为微针射频侧与超脉冲CO_(2)点阵激光侧。治疗间隔2个月,共治疗2次,记录治疗后患者的不良反应、患者自评疼痛度及满意度。由医生通过分析治疗前、后患者面部影像数据评估疗效。结果:经过2次治疗,所有患者面部双侧痤疮瘢痕均有不同程度改善,痤疮瘢痕权重评分均下降(P<0.01)。对于V形、M形瘢痕,微针射频侧疗效优于CO_(2)点阵激光侧(P<0.05)。对于U形瘢痕,两种治疗疗效差异无统计学意义(P>0.05)。与CO_(2)点阵激光侧相比,微针射频侧治疗疼痛评分更高(P<0.01),不良反应更少。结论:微针射频与CO_(2)点阵激光均能有效改善面部萎缩性痤疮瘢痕,微针射频不良反应更少。对于U形瘢痕,两种治疗疗效相近,而对于较深的V形瘢痕和较大的M形瘢痕,微针射频疗效更显著。Objective:To compare the efficacy and safety of invasive microneedle radiofrequency(MRF)versus ultrapulsed fractional CO_(2)laser for the treatment of facial atrophic acne scars through a prospective,split-face study.Methods:Twentysix patients with facial atrophic acne scars were enrolled.Each patient randomly received invasive MRF on one half of the face,and ultrapulsed fractional CO_(2)laser on the other facial side once every two months for two sessions.Adverse reactions were recorded after treatment,and the degrees of pain and satisfaction were self-evaluated,while the efficacy was evaluated and analyzed with patients'facial imaging data by doctors.Results:All patients showed different degrees of improvement in acne scars of bilateral face and reduction inéchelle devaluation clinique descicatrices dacne(ECCA)scores(P<0.01)after two sessions of treatments.For V-and M-shaped scars,the MRF side was more effective than the fractional CO_(2) laser side(P<0.05).For U-shaped scars,the difference in efficacy between the two treatments was not significant(P>0.05).Compared with the side receiving CO_(2)fractional laser,the side receiving MRF had a lower pain score(P<0.01)and fewer adverse effects.Conclusion:MRF and ultrapulsed CO_(2)fractional laser are both effective in improving facial atrophic acne scars,while MRF shows fewer adverse reactions.For U-shaped scars,the efficacy of both treatments is similar.For deep V-shaped scars and large M-shaped scars,MRF is more effective.
