摘 要:假体周围关节感染(PJI)是全关节置换术后最常见的并发症之一。它难以管理,具有严重的发病率和病死率,为卫生保健系统带来了沉重的经济负担。而细菌生物膜的形成使PJI治疗更加具有挑战性,生物膜的存在使得病原菌对抗生素和宿主免疫反应产生了屏障。噬菌体作为一种裂解细菌的病毒,因其具有治疗耐药细菌感染的潜力而备受关注。尽管噬菌体疗法(PT)前景广阔,但其在PJI管理中的疗效尚不清楚。目前的大部分文献主要局限于病例报告和系列报道。该文基于相关病例报告叙述了关节置换术后PJI的发生现状、对生物膜形成的理解以及生物膜形成促进免疫逃逸的机制等,探讨噬菌体疗法治疗PJI的应用现状、疗效与不足、应用前景,以期为PJI的治疗提供新的途径。Periprosthetic joint infection(PJI)is one of the most common complications after total joint replacement.It is difficult to manage,has severe morbidity and mortality,and places a heavy economic burden on the health care system.The formation of bacterial biofilms makes PJI treatment more challenging,and the presence of biofilms creates a barrier for pathogenic bacteria to resist antibiotics and host immune responses.Bacteriophages,as viruses that lyse bacteria,have attracted much attention due to their potential to treat drug-resistant bacterial infections.Despite the promise of phage therapy(PT),its efficacy in the management of PJI is unclear.Much of the current literature is largely confined to case reports and series.Based on relevant case reports,this article discusses the application status,efficacy,shortcomings and application prospects of PT in the treatment of PJI,so as to provide a new approach for the treatment of PJI.
