杂合式血液净化方案治疗老年慢性肾衰竭患者的有效性与安全性

摘　　要：目的探析杂合式血液净化方案在老年慢性肾衰竭患者治疗中的应用。方法将2020年1月至2022年12月收治的100例老年慢性肾衰竭患者以随机法分为常规组(50例,常规血液净化方案治疗)和观察组(50例,杂合式血液净化方案治疗)。比较两组的治疗效果。结果观察组的治疗总有效率高于常规组(P<0.05)。治疗后,观察组的血肌酐(Scr)、血尿素氮(BUN)、甲状旁腺激素(PTH)及β_(2)-微球蛋白(β_(2)-MG)水平低于常规组,内生肌酐清除率(Ccr)水平高于常规组(P<0.05)。治疗后,观察组的钙(Ca)、骨形态形成蛋白-7(BMP-7)水平高于常规组,血磷(P)、成纤维细胞生长因子-23(FGF-23)、镁(Mg)、结缔组织生长因子(CTGF)水平低于常规组(P<0.05)。观察组的不良反应总发生率低于常规组(P<0.05)。结论杂合式血液净化方案治疗老年慢性肾衰竭效果显著,可有效提高中、大分子毒素物质的清除效果,改善患者肾功能与骨代谢指标,且治疗安全性较高,值得推广。Objective To explore the application of heterozygous blood purification regimen in the treatment of elderly patients with chronic renal failure.Methods A total of 100 elderly patients with chronic renal failure admitted from January 2020 to December 2022 were divided into conventional group(50 cases,routine blood purification regimen)and observation group(50 cases,heterozygous blood purification regimen)by random method.The therapeutic effects of the two groups were compared.Results The total effective rate of treatment in the observation group was higher than that in the conventional group(P<0.05).After treatment,the levels of serum creatinine(Scr),blood urea nitrogen(BUN),parathyroid hormone(PTH)and β_(2)-microglobulin(β_(2)-MG)in the observation group were lower than those in the conventional group,and the level of endogenous creatinine clearance rate(Ccr)was higher than that in the conventional group(P<0.05).After treatment,the levels of calcium(Ca)and bone morphogenetic protein-7(BMP-7)in the observation group were higher than those in the conventional group,and the levels of phosphate(P),fibroblast growth factor-23(FGF-23),magnesium(Mg)and connective tissue growth factor(CTGF)were lower than those in the conventional group(P<0.05).The total incidence of adverse reactions in the observation group was lower than that in the conventional group(P<0.05).Conclusion The heterozygous blood purification regimen has a significant effect in the treatment of elderly patients with chronic renal failure.It can effectively improve the clearance effect of medium and large molecular toxins,ameliorate the renal function and bone metabolism indexes of patients,and has high treatment safety,which is worthy of promotion.
