围手术期间歇性充气加压对重度颅脑损伤患者下肢深静脉血栓形成的预防效果

摘　　要：目的分析重度颅脑损伤(TBI)患者围手术期应用间歇性充气加压对下肢深静脉血栓的预防效果。方法选取2021-01—2022-12菏泽市立医院收治的70例重度TBI患者为研究对象,分为对照组、研究组各35例,对照组接受常规下肢深静脉血预防性干预,研究组增加间歇性充气加压。比较2组下肢深静脉血栓发生率、干预前后静脉血流流速及干预7 d后下肢肿胀情况。结果研究组发生下肢深静脉血栓1例(2.86%),对照组6例(17.14%),研究组发生率低于对照组(P<0.05);干预前2组下肢各血管静脉流速对比无统计学差异(P>0.05),干预后2组股静脉、髂外静脉、股深静脉、腘静脉流速较干预前均明显提升,研究组流速快于对照组(P<0.05);对照组、研究组轻度+无肿胀占比差异有统计学意义(P<0.05)。结论间歇性充气加压可有效预防肢深静脉血栓,原因可能在于间歇性充气加压可改善下肢血管静脉流速,建议重度TBI患者围术期应用。Objective To analyze the preventive effect of intermittent inflation and compression on deep vein thrombosis of the lower limbs in patients with severe traumatic brain injury(TBI)during the perioperative period.Methods Seventy patients with severe TBI in the Heze Municipal Hospital from January 2021 to December 2022 were selected as the study subjects.They were divided into the control group and the study group,with 35 patients in each group.The control group received routine preventive intervention for lower limb deep venous blood,while the study group received intermittent inflation and compression.The incidence of lower limb deep vein thrombosis,venous blood flow velocity before and after intervention,and lower limb swelling after seven days of intervention between the two groups were compared.Results The incidence of deep vein thrombosis in the lower limbs in the study group was 1 case(2.86%),while in the control group it was 6 cases(17.14%),the incidence rate in the study group was lower than that in the control group(P<0.05).Before intervention,there was no significant difference(P>0.05)in the flow velocity of the lower limb blood vessels between the two groups.After intervention,the flow velocity of the femoral vein,external iliac vein,deep femoral vein,and popliteal vein in both groups was significantly increased compared to before intervention,and the flow velocity in the study group was faster than that in the control group(P<0.05).There was a statistically significant difference in the proportion of mild and no swelling between the control group and the study group(P<0.05).Conclusion The application of intermittent inflation and compression during the perioperative period in patients with severe TBI can effectively prevent deep vein thrombosis in the limbs.The reason may be that intermittent inflation and compression can improve the flow velocity of lower limb blood vessels and veins.It is recommended to use it during the perioperative period.
