摘 要:目的:观察体外冲击波疗法联合干扰电治疗膝骨关节炎(knee osteoarthritis,KOA)的临床疗效。方法:选取2021年1月—2023年1月在上海市徐汇区田林街道社区卫生服务中心康复科就诊的KOA患者140例,随机分成两组,每组70例。对照组采用干扰电治疗,30 min/次,1次/d,5次/周,治疗4周;治疗组在对照组基础上联合体外冲击波治疗,1次/周,共治疗4周。分别采用视觉模拟量表(visual analogue scale,VAS)、西安大略和麦克马斯特大学骨关节炎指数(Western Ontario and McMaster University osteoarthritis index,WOMAC)和膝关节功能评分量表(knee joint function scale,Lysholm)评价两组患者治疗前后膝关节疼痛程度及功能。结果:治疗4周后,两组的VAS评分、WOMAC评分和Lysholm评分均较治疗前显著改善(P<0.01);两组治疗前后VAS差值、WOMAC差值和Lysholm差值间的差异均具有统计学意义(P<0.01)。结论:相比于干扰电单一治疗,体外冲击波联合干扰电治疗能更好地缓解KOA患者膝关节疼痛、肿胀与活动受限症状,明显改善患者膝关节功能。Objective:To observe the clinical effect of extracorporeal shock wave therapy combined with interference electrotherapy in the treatment of knee osteoarthritis(KOA).Methods:A total of 140 patients with KOA who visited the Rehabilitation Department of Tianlin Community Health Service Center of Xuhui District,Shanghai from January 2021 to January 2023 were selected and randomly divided into two groups with 70 cases in each group.The control group was treated with interference electrotherapy,30 minutes per time,once a day,5 times per week,and the treatment was for four weeks;the treatment group was treated with extracorporeal shock wave therapy on the basis of the control group,once a week,and received a 4-week treatment.The visual analogue scale(VAS),Western Ontario and McMaster University osteoarthritis index(WOMAC)and knee joint function scale(Lysholm)were used to evaluate the degree of knee joint pain and function of two groups before and after treatment.Results:After 4 weeks of treatment,VAS scores,WOMAC scores and Lysholm scores in both groups were significantly improved compared with those before treatment(P<0.01);the differences of VAS,WOMAC and Lysholm before and after treatment between the two groups were statistically significant(P<0.01).Conclusion:Compared with interference electrotherapy alone,extracorporeal shock wave therapy combined with interference electrotherapy can alleviate the symptoms of knee joint pain,swelling and limited mobility in patients with KOA better,and significantly improve the knee joint function of patients.
