摘 要:目的:探讨高分辨率CT用于不同性质孤立性肺小结节临床诊断中的作用,着重分析良恶性患者的结节边缘形态、内部特征。方法:2023年1月至2024年1月,以发生孤立性肺小结节病例100例为研究对象,均在医院完成高分辨率CT诊断及活检/手术病理学诊断,以后者为金标准,统计良性、恶性两种结节的发生病例数,同时统计高分辨率CT诊断与金标准诊断的一致性,计算诊断效能,对比良性、恶性病例的结节边缘形态与内部特征表现、各增强扫描时间CT值。结果:100例患者均经金标准确诊疾病,确诊率100.00%;高分辨率CT的诊断敏感度是97.14%、特异度是96.67%、准确性是97.00%。对比良性结节70例、恶性结节30例的结节边缘形态与内部特征表现、各增强扫描时间CT值,均存在统计学差异(P<0.05)。结论:对孤立性肺小结节实施高分辨率CT检查,诊断准确性、敏感度及特异度均比较高,还可鉴别不同性质结节患者的边缘形态、内部特征,通过增强扫描CT值辅助诊断,应用价值显著。Objective:To explore the role of high-resolution CT in the clinical diagnosis of different solitary pulmonary nodules,focusing on the analysis of marginal morphology and internal features of nodules in benign and malignant patients.Methods:From January 2023 to January 2024,100 cases of isolated small pulmonary nodules were selected as subjects.High-resolution CT diagnosis and biopsy/surgical pathology diagnosis were completed in the hospital.The latter was used as the gold standard to calculate the number of cases of benign and malignant nodules,and the consistency between high-resolution CT diagnosis and gold standard diagnosis was calculated.The edge morphology,internal features and CT values of each enhanced scan time were compared between benign and malignant cases.Results:100 patients were diagnosed by gold standard,the diagnosis rate was 100.00%;The diagnostic sensitivity,specificity and accuracy of highresolution CT were 97.14%,96.67%and 97.00%.Six items of marginal morphology and internal features of 70 benign nodules and 30 malignant nodules,and four items of enhanced scanning time CT values were compared,and the results showed that(P<0.05).Conclusion:High-resolution CT diagnosis of isolated pulmonary nodules has high diagnostic accuracy,sensitivity and specificity,and can also distinguish the marginal morphology and internal characteristics of patients with different nodule characteristics.Enhanced CT scan value is of significant application value.
