摘 要:目的:分析超声对早孕期胎儿中枢神经系统畸形检查的意义,为早孕期胎儿中枢神经系统畸形检查提供参考。方法:选取我院2019年9月至2020年9月1784例早孕期的孕妇并对其进行超声检查,比较不同孕周中枢神经系统及侧脑室水平的显示,并比较早孕期超声检查结果与金标准。结果:孕11~11^(+6)周、孕12~12^(+6)周、孕13~13^(+6)周的中枢神经系统头胸部正中矢状断面、脊柱长轴断面显示比较变化不明显(P>0.05)。孕11~11^(+6)周、孕12~12^(+6)周、孕13~13^(+6)周孕妇的中枢神经系统侧脑室水平横断面显示率、小脑与颅后窝池断面显示率比较变化不明显(P>0.05)。在全部孕妇中胎儿存在中枢神经系统畸形率为0.45%,灵敏度和特异性分别为90.00%、100%,阳性和阴性预测值各为100%、99.94%。结论:早孕期超声筛查儿中枢神经系统畸形具有较高的显示率,可有效筛查不同的畸形情况,预测阳性疾病,并可指导临床治疗工作的开展。Objective:To analyze the significance of ultrasound in the examination of fetal central nervous system malformation in early pregnancy,and to provide reference for the examination of fetal central nervous system malformation in early pregnancy.Methods:1784 cases of pregnant women in early pregnancy in our hospital from September 2019 to September 2020 were selected and examined by ultrasound.The levels of central nervous system and lateral ventricle in diferent gestational weeks were compared,and the results of ultrasound examination in early pregnancy were compared with the gold standard.Results:The median sagittal sections of the head and chest and the long axis of the spine of the central nervous system at weeks 11~11^(+6),12~12^(+6) and 13~13^(+6) showed no obvious changes(P>0.05).There were no significant changes in thelateral ventricle and cerebellum and posterior fosa cisten in pregnant women at 11 to 11^(+6)weeks,12 to 12^(+6) weeks and 13 to 13^(+6) weeks of gestation(P>0.05).In all pregnant women,the fetal central nervous system malformation rate was 0.45%,the sensitivity and specificity were 90.00%and 100%,respectively,and the positive and negative predictive values were 100%and 99.94%respectively.Conclusion:Ultrasound screening for central nervous system malformation in early pregnancy has a high display rate,which can effectively screen different malformations,predict positive diseases,and guide the development of clinical treatment.
