摘　　要：目的:系统评价并整合危重患儿医用黏胶相关性皮肤损伤(MARSI)管理的最佳证据。方法:检索中国知网、万方数据库、维普数据库、PubMed、the Cochrane Library等数据库及BMJ Best Practice、澳大利亚Joanna Briggs Institute(JBI)循证卫生保健研究中心、英国国家卫生与临床优化研究所(NICE)、中国指南网等网站的专家共识、专家意见及指南,采用澳大利亚JBI循证卫生保健中心的文献评价标准和证据分级系统对不同类型的研究进行文献质量评价和证据级别评定。结果:共纳入14篇文献,包括2篇专家意见、4篇专家共识和8篇指南,总结了包括教育与培训、评估与监测、皮肤保护、实施要点、更换敷料时机在内的25条预防危重患儿MARSI的最佳证据。结论:医护人员应基于循证证据并结合危重患儿的实际情况规范医用黏胶的使用,以减少MARSI的发生。Objective:To systematically evaluate and integrate the best evidence for the management of medical adhesive-related skin injury(MARSI)in critically ill children.Methods:We searched the CNKI,WanFang Database,VIP,PubMed,the Cochrane Library,and other databases,as well as websites such as BMJ Best Practice,Joanna Briggs Institute(JBI)Evidence-Based Health Care Research Centre,National Institute for Health and Clinical Excellence,and China Guidelines.We evaluated the quality of the literature and the level of evidence of different types of studies by using the literature evaluation criteria and evidence grading system of the JBI Center for Evidence-Based Health Care in Australia,as well as the expert consensus,expert opinion and guidelines.Results:A total of 14 articles,including 2 expert opinions,4 expert consensus and 8 guidelines,were included in the study,which summarized the 25 best evidence for the prevention of MARSI in critically ill children,including education and training,assessment and monitoring,skin protection,implementation points,and the timing of dressing replacement.Conclusion:Healthcare professionals should standardize the use of medical adhesives based on evidence-based evidence and in the context of critically ill children to reduce the incidence of MARSI.
