尿毒症患者血清蛋白电泳指标、NT-proBNP和Hb浓度的变化

摘　　要：目的探讨血清蛋白Alb、α_(1)-球蛋白、α_(2)-球蛋白、β_(1)-球蛋白、β_(2)-球蛋白和γ-球蛋白百分比、NT-proBNP和Hb浓度在尿毒症患者中的变化。方法选取2022年1月至2023年8月于北京航天总医院血透中心治疗的136例尿毒症透析前患者作为试验组A组,以及50例健康体检者作为对照组B组,对比2组基本资料及血清蛋白Alb、α_(1)-球蛋白、α_(2)-球蛋白、β_(1)-球蛋白、β_(2)-球蛋白和γ-球蛋白百分比、Hb和NT-proBNP浓度,采用t检验分析以上指标在疾病中的变化。结果与对照组B组比较,A组更容易出现α_(1)-球蛋白(P<0.0001)和γ-球蛋白(P<0.0001)百分比升高;Alb(P<0.01)和β_(1)-球蛋白(P<0.0001)百分比下降;α_(2)-球蛋白(P>0.05)和β_(2)-球蛋白(P>0.05)百分比未发现明显变化;NT-proBNP(P<0.0001)水平显著升高;Hb浓度(P<0.0001)显著下降。结论尿毒症患者的血清α_(1)-球蛋白百分比、γ-球蛋白百分比和NT-proBNP浓度升高;Alb百分比、β_(1)-球蛋白百分比和Hb浓度下降。Objective To evaluate changes of the percentages of serum protein Alb,α_(1)-globulin,α_(2)-globulin,β_(1)-globulin,β_(2)-globulin andγ-globulin,also concentrations of NT-proBNP and hemoglobin in uremic patients.Methods We selected 136 uremic patients who maintain hemodialysis at Beijing Aerospace General Hospital from January,2022 to August,2023 for this study.We compared selected patients with 50 patients from medical examination to detect the changes of the percentages of serum protein Alb,α_(1)-globulin,α_(2)-globulin,β_(1)-globulin,β_(2)-globulin andγ-globulin,also concentrations of NT-proBNP and hemoglobin,by using t test.Results By analyzing the serum protein electrophoresis of 136 individuals in experimental group A and 50 individuals in control group B,we found that group A had higher levels ofα_(1)-globulin(P<0.0001)andγ-globulin(P<0.0001),while lower levels of Alb(P<0.01)andβ_(1)-globulin(P<0.0001).No significant differences were found in the content ofα_(2)-globulin(P>0.05)andβ_(2)-globulin(P>0.05).We also discovered that NT-proBNP(P<0.0001)increased,while Hb concentration(P<0.0001)decreased significantly.Conclusion In serum of uremic patients,the percentages ofα_(1)-globulin,γ-globulin and the concentration of NT-proBNP increase,while percentages of Alb,β_(1)-globulin and concentration of Hb decrease.
