摘　　要：目的探讨血清白介素-6(IL-6)、中性粒细胞/淋巴细胞比值(NLR)、血小板/淋巴细胞比值(PLR)和红细胞沉降率(ESR)水平在成人社区获得性肺炎(CAP)诊治中的应用价值。方法收集2022年4月至2023年1月在本院住院的110例CAP确诊患者作为CAP组,其中非重症80例,重症30例,再选择同一时期在我院进行体检的90例健康人群作为对照组。比较基本信息及IL-6、NLR、PLR和ESR等实验室指标在CAP组与对照组、CAP轻重症之间的差异,并进行统计学分析。结果CAP组的IL-6、NLR、PLR和ESR水平均显著高于对照组(P<0.05);CAP重症组的IL-6、NLR和PLR水平均显著高于非重症组(P<0.05),ESR水平差异无统计学意义(P>0.05)。ROC曲线结果显示,IL-6、NLR、PLR和ESR联合检测在CAP诊断中的AUC为0.949(95%CI 0.908~0.975),高于4项指标单独检测。结论IL-6、NLR、PLR和ESR在成人CAP的血清内表达水平较高,4项指标联合检测可有效提高CAP诊断准确度,IL-6、NLR、PLR水平与CAP病情严重程度呈相关性,可以为临床诊断提供依据。Objective To investigate the application value of serum interleukin-6(IL-6),neutrophil/lymphocyte ratio(NLR),platelet/lymphocyte ratio(PLR)and erythrocyte sedimentation rate(ESR)levels in the diagnosis and treatment of adult community-acquired pneumonia(CAP).Methods A total of 110 patients with confirmed CAP admitted to our hospital from April,2022 to January,2023 were selected as the CAP group,including 80 non-severe cases and 30 severe cases,while 90 healthy subjects in our hospital during the same period were selected as the control group.The basic information of these two groups and the differences between the two groups of patients for multiple indexes,including IL-6,NLR,PLR and ESR were statistically compared and evaluated.Results Levels of IL-6,NLR,PLR and ESR in the CAP group were significantly higher than those in the control group(P<0.05),while levels of IL-6,NLR and PLR in the severe CAP group were significantly higher than those in the non-severe group(P<0.05),and there was no significant difference in ESR levels(P>0.05).ROC curve showed that the AUC of the combination of IL-6,NLR,PLR and ESR in the diagnosis of CAP was 0.949(95%CI 0.908-0.975),which was higher than that of each of four indicators individually.Conclusion Serum expression levels of IL-6,NLR,PLR and ESR in adults with CAP are high,and the combined detection of these four indicators could effectively improve the diagnostic accuracy of CAP,and levels of IL-6,NLR and PLR are correlated with the severity of CAP,which could provide a reference basis for the clinical diagnosis.
