人工关节置换术与内固定术治疗骨质疏松性不稳定股骨粗隆间骨折的效果观察

摘　　要：目的:探讨人工关节置换术与内固定术治疗骨质疏松性不稳定股骨粗隆间骨折(ITF)的临床效果。方法:选取2022年4月—2023年4月核工业四一七医院收治的骨质疏松性不稳定ITF患者66例作为研究对象,随机分为对照组(实施内固定术)和观察组(应用人工关节置换术),各33例。比较两组手术指标、术后恢复效果、髋关节功能、骨折愈合效果及术后并发症发生率。结果:观察组手术操作用时短于对照组,术中出血量、输血量多于对照组,差异有统计学意义(P<0.05);观察组关节功能恢复时间、术后卧床时间、住院时间均短于对照组,差异有统计学意义(P<0.05);术后1个月,两组Harris髋关节功能评分高于术前,观察组高于对照组,差异有统计学意义(P<0.05);观察组骨折愈合优良率高于对照组,差异有统计学意义(P=0.001);观察组术后并发症发生率低于对照组,差异有统计学意义(P<0.001)。结论:人工关节置换术治疗骨质疏松性不稳定ITF的临床效果优于内固定术,其手术用时更短,可加快患者髋关节功能恢复及骨折愈合,且安全性较高,但术中出血量与输出量较多。Objective:To investigate the clinical effect of artificial arthroplasty and internal fixation in the treatment of osteoporotic unstable intertrochanteric fracture of femur(ITF).Methods:Sixty-six patients with osteoporotic unstable ITF admitted to the Nuclear Industry 417 Hospital from April 2022 to April 2023 were selected as the study subjects,and were randomly divided into the control group(implementation of internal fixation)and the observation group(application of artificial arthroplasty),with thirty-three cases in each group.Compare the surgical index,postoperative recovery effect,hip joint function,fracture healing effect and postoperative complication rate of the two groups.Results:The observation group took shorter time for surgical operation than the control group,and intraoperative bleeding and blood transfusion were more than the control group,the difference was statistically significant(P<0.05);the observation group had shorter recovery time of joint function,postoperative bed resting time and hospitalization time than the control group,the difference was statistically significant(P<0.05);one month after surgery,the Harris hip function score of both groups was higher than that of the preoperative period,and the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05);the excellent rate of fracture healing of the observation group was higher than that of the control group,and the difference was statistically significant(P=0.001);the incidence of postoperative complications in the observation group was lower than that of the control group,and the difference was statistically significant(P<0.001).Conclusion:The clinical effect of artificial joint replacement for osteoporotic unstable ITF is better than that of internal fixation,and its operation time is shorter,which can accelerate the recovery of patients'hip joint function and fracture healing,but the amount of intraoperative bleeding and output is higher.
