经阴道彩色多普勒超声在卵巢黄体破裂与宫外孕破裂中的诊断作用

摘　　要：目的:分析经阴道彩色多普勒超声在卵巢黄体破裂与宫外孕破裂中的诊断作用。方法:选取2022年1月—2023年12月于盘州市人民医院经手术病理确诊为宫外孕破裂患者50例和卵巢黄体破裂患者45例作为研究对象,分别设为宫外孕破裂组和卵巢黄体破裂组。所有患者接受经阴道彩色多普勒超声检查,记录并分析超声图像中的血流特征、血流动力学指标。结果:黄体破裂超声表现为卵巢增大,混合回声包块,盆腔积液,见黄体囊肿伴环状血流;宫外孕破裂超声表现为宫旁包块,胎心少见,破裂出血见腹腔积液,子宫被液暗区包围。两组多普勒表现、血液频谱特征比较,差异有统计学意义(P<0.05);卵巢黄体破裂组阻力指数和搏动指数低于宫外孕破裂组,收缩期峰值流速高于宫外孕破裂组,差异有统计学意义(P<0.001)。结论:经阴道彩色多普勒超声可以通过不同超声表现、血流特征、血流动力学指标鉴别诊断卵巢黄体破裂与宫外孕破裂。Objective:To analyze the diagnostic value of transvaginal color Doppler ultrasound in ruptured corpus luteum of ovary and ruptured ectopic pregnancy.Methods:Fifty patients with ruptured ectopic pregnancy and forty-five patients with ruptured corpus luteum of ovary confirmed by surgery and pathology in Panzhou People's Hospital from January 2022 to December 2023 were selected as the study objects,and were set as the ruptured ectopic pregnancy group and the ruptured corpus luteum of ovary group,respectively.All patients underwent transvaginal color Doppler ultrasound,and the blood flow characteristics and hemodynamics indexes in the ultrasound images were recorded and analyzed.Results:Luteal rupture ultrasound showed enlarged ovary,mixed echo mass,pelvic effusion,luteal cyst with annular blood flow;ectopic pregnancy rupture ultrasound showed parafunctal mass,fetal heart was rare,rupture bleeding was abdominal fluid,and the uterus was surrounded by liquid dark area;there were significant differences in Doppler and blood spectrum between the two groups(P<0.05);the resistance index and pulsation index of the ruptured corpus luteum group were lower than those of the ruptured ectopic pregnancy group,and the peak systolic flow rate was higher than that of the ruptured ectopic pregnancy group,the difference was statistically significant(P<0.001).Conclusion:Transvaginal color Doppler ultrasound can distinguish the rupture of corpus luteum of ovary from rupture of ectopic pregnancy by different ultrasonic manifestations,blood flow characteristics and hemodynamics.
