从脾阴论治糖尿病伴失眠

摘　　要：临床上2型糖尿病患者常伴见失眠。糖尿病以阴虚内热为基本病机,病位在脾、多为脾阴虚。脾阴由脾脏运化所产生的水谷精微而组成,脾阴虚则脾脏运化功能失常。脾阴虚是糖尿病的始动环节。脾阴不足、营卫亏虚,脾阴不足、心失所养,脾阴不足、脾胃升降失调均可致失眠。临床治疗以酸甘平补滋营阴为主,同时予升提散阴养心神、斡旋气机交心肾之法。并结合临床验案对从脾阴论治糖尿病伴失眠进行阐述。Type 2 diabetes patients often experience insomnia,commonly linked to yin deficiency and internal heat,with the primary pathology located in the spleen,frequently manifesting as spleen-yin deficiency.Spleen-yin,composed of the essence derived from the spleen’s transformation of food and fluids,is crucial for normal spleen function.Spleen-yin deficiency disrupts this function,marking the onset of diabetes.Insomnia in these patients can result from spleen-yin deficiency leading to the deficiency of the ying and wei qi,disharmony in the regulation of the heart,and imbalance between the spleen and stomach’s ascending and descending functions.Clinical treatment primarily involves using sour,sweet,and neutral herbs to nourish the yin,along with method to elevate and disperse yin,nourish the mind,harmonize qi,and mediate the heart and kidney.This paper discusses the treatment of diabetes mellitus with insomnia from the perspective of spleen-yin theory,supported by clinical case studies.
