组合式外固定器与内固定技术在复杂四肢骨折患者中的对比效果

摘　　要：目的:对比组合式外固定器与内固定技术在复杂四肢骨折患者中的效果。方法:选取2021年4月—2023年4月漳州市中医院收治的88例复杂四肢骨折患者作为研究对象,依据治疗方案不同分为研究组与对照组,每组各44例。研究组行组合式外固定器治疗,对照组行内固定技术治疗,比较两组手术效果、并发症状况以及围术期疼痛度、创伤应激情况。结果:研究组手术总有效率高于对照组,差异有统计学意义(P<0.05);研究组术后1 d、3 d、7 d的视觉模拟量表(VAS)评分均低于对照组,差异有统计学意义(P<0.05);研究组术后6个月上肢与下肢Fugel-Meyer运动功能评定(FMA)评分均高于对照组,差异有统计学意义(P<0.05);研究组术后3 d血清肾上腺素(E)、去甲肾上腺素(NE)、皮质醇(Cor)水平均低于对照组,差异有统计学意义(P<0.05);研究组并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论:组合式外固定器应用于复杂四肢骨折患者中,可有效改善手术效果,缓解术后疼痛度,有助于肢体功能恢复,抑制创伤应激,并保障其安全性。Objective:To compare the effect of combined external fixation and internal fixation in patients with complex limb fractures.Method:A total of 88 patients diagnosed with complex limb fracture in the hospital from April 2021 to April 2023 were retrospectively collected and divided into study group and control group according to different treatment plans,with 44 cases in each group.The study group was treated with combined external fixation,and the control group was treated with internal fixation.The operative effect,complications,perioperative pain and traumatic stress were compared between the two groups.Result:The total effective rate of the study group was higher than that of the control group,the difference was statistically significant(P<0.05).VAS scores of the study group were lower than those of the control group 1d,3d and 7d after surgery,The difference was statistically significant(P<0.05).FMA scores of the upper and lower limbs of the study group were higher than those of the control group 6 months after surgery,The difference was statistically significant(P<0.05).Serum E,NE and Cor levels of the study group 3d after surgery were lower than those of the control group,with statistical significance(P<0.05).The complication rate in the study group was significantly lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion:The application of combined external fixator in patients with complex fracture of limbs can effectively improve the surgical effect,relieve postoperative pain,benefit the recovery of limb function,inhibit traumatic stress,and ensure safety.
