摘 要:目的:探讨MRI与CT诊断重型高血压脑出血术后早期脑缺血的价值。方法:选取2021年1月—2023年12月南华大学附属南华医院收治的94例疑似重型高血压脑出血术后早期脑缺血患者。所有患者均接受MRI与CT检查。以血管造影结果为金标准,分析CT检查、MRI检查脑缺血检出情况。分析CT检查、MRI检查的诊断效能。比较阳性与阴性患者表观扩散系数(ADC)值、脑血流量(CBF)、脑血容量(CBV)、对比剂平均通过时间(MTT)。结果:以血管造影结果为金标准,CT检查真阳性45例、真阴性25例;MRI检查真阳性57例、真阴性34例。MRI检查敏感度、准确度、特异度均高于CT检查,差异有统计学意义(P<0.05)。阳性患者ADC值、CBF、CBV均低于阴性患者,MTT长于阴性患者,差异有统计学学意义(P<0.05)。结论:在重型高血压脑出血术后早期脑缺血诊断中MRI、CT均有一定价值,但MRI检查准确率更高。Objective:To explore the value of MRI and CT in diagnosing early cerebral ischemia after severe hypertensive intracerebral hemorrhage surgery.Method:A total of 94 patients with suspected early cerebral ischemia after severe hypertensive cerebral hemorrhage surgery were selected from Affiliated Nanhua Hospital,University of South China from January 2021 to December 2023.All patients underwent MRI and CT examination.The results of angiography were used as the gold standard to analyze the cerebral ischemia detection situation of CT and MRI examination.The diagnostic efficacy of CT and MRI examination were analyzed.Apparent diffusion coefficient(ADC)value,cerebral blood flow(CBF),cerebral blood volume(CBV)and mean transit time(MTT)of contrast agent were compared between positive and negative patients.Result:With angiography as the gold standard,45 cases were true positive and 25 were true negative cases on CT.There were 57 true positive cases and 34 true negative cases on MRI.The sensitivity,accuracy and specificity of MRI examination were higher than those of CT examination,the difference was statistically significant(P<0.05).ADC value,CBF and CBV in positive patients were lower than those in negative patients,and MTT was longer than that in negative patients,the differences were statistically significant(P<0.05).Conclusion:Both MRI and CT examination have certain value in early cerebral ischemia diagnosis after severe hypertensive cerebral hemorrhage surgery,but MRI has higher accuracy.
