摘　　要：目的:探析氨甲环酸联合诊断性刮宫术治疗围绝经期异常子宫出血(AUBP)的效果。方法:选择2021年6月—2023年6月在北京怀柔医院治疗的90例AUBP患者作为研究对象,使用随机数表法将其分为对照组(诊断性刮宫术+炔诺酮)与研究组(诊断性刮宫术+氨甲环酸),各45例。比较两组临床疗效、血红蛋白(Hb)、子宫内膜厚度、性激素[雌二醇(E_(2))、促卵泡刺激素(FSH)、促黄体生成素(LH)、睾酮(T)]、炎症因子[白细胞介素-2(IL-2)、肿瘤坏死因子-α(TNF-α)]、血管内皮生长因子(VEGF)和碱性成纤维细胞生长因子(bFGF)。结果:研究组治疗总有效率高于对照组,差异有统计学意义(P<0.05)。治疗后,两组Hb、VEGF、bFGF较治疗前升高,子宫内膜厚度、E_(2)、FSH、LH、T、IL-2、TNF-α较治疗前降低,且研究组改善优于对照组,差异有统计学意义(P<0.05)。结论:AUBP患者接受氨甲环酸联合诊断性刮宫术治疗,效果显著,能降低子宫内膜厚度、炎症因子水平,改善性激素、Hb、VEGF、bFGF。Objective:To explore the effect of Tranexamic Acid combined with diagnostic curettage in the treatment of abnormal uterine bleeding perimenopausal(AUBP).Method:A total of 90 patients with AUBP who treated in Beijing Huairou Hospital from June 2021 to June 2023 were selected as the study objects,and they were randomly divided into the control group(diagnostic curettage+Norethindrone)and the study group(diagnostic curettage+Tranexamic Acid),with 45 cases in each group.The clinical efficacy,hemoglobin(Hb),endometrial thickness,sex hormones[estradiol(E_(2)),follicle stimulating hormone(FSH),luteinizing hormone(LH),testosterone(T)],inflammatory factors[interleukin-2(IL-2),tumor necrosis factor-α(TNF-α)],vascular endothelial growth factor(VEGF)and basic fibroblast growth factor(bFGF)were compared between two groups.Result:The total effective rate of treatment in the study group was higher than that in the control group,the difference was statistically significant(P<0.05).After treatment,the Hb,VEGF and bFGF in two groups were higher than those before treatment,the endometrial thickness,E_(2),FSH,LH,T,IL-2 and TNF-αwere lower than those before treatment,and the improvements in the study group were better than those in the control group,the differences were statistically significant(P<0.05).Conclusion:AUBP patients receiving Tranexamic Acid combined with diagnostic curettage treatment have significant therapeutic effects,which can reduce endometrial thickness,inflammatory factor levels,improve sex hormone,Hb levels,VEGF and bFGF.
