自制润肠灵联合生物反馈治疗功能性出口梗阻型便秘疗效评价

摘　　要：目的探究自制润肠灵联合生物反馈治疗功能性出口梗阻型便秘(FOOC)的疗效。方法选取2020年2月至2022年2月常州市中医医院收治的83例FOOC患者,根据随机数字表法分为2组:对照组41例,采用生物反馈治疗;观察组42例,在生物反馈治疗的基础上给予自制润肠灵治疗。治疗4周后,比较2组患者疗效、中医症状评分、肛门动力学指标、排便时间、排便间隔时间、72 h排便次数、治疗后3个月内FOOC的复发情况。结果观察组与对照组FOOC患者治疗有效率分别为92.86%(39/42)、73.17%(30/41),观察组高于对照组(P<0.05)。治疗后,2组FOOC患者中医主症评分、次症评分及总评分均降低,且观察组各项评分低于对照组(P<0.05)。2组FOOC患者肛管最大收缩压(AMCP)、肛管静息压(ARP)水平降低,肛管持续挤压时间缩短(P<0.05),观察组AMCP、ARP及肛管持续挤压时间短于对照组(P<0.05)。观察组FOOC患者排便时间、排便间隔时间均短于对照组,72 h排便次数多于对照组(P<0.05)。观察组治疗后3个月内4例复发,复发率为9.52%,对照组11例复发,复发率为26.83%,观察组复发率低于对照组(χ2=4.196,P=0.041)。结论自制润肠灵联合生物反馈治疗能够提高FOOC患者的疗效,改善中医症状及肛门动力学指标,缩短排便间隔及排便时间,增加排便次数,降低复发率。Objective To explore the efficacy of self⁃made Runchangling combined with biofeedback in the treatment of functional outlet obstruction constipation(FOOC).Methods A total of 83 FOOC patients who were admitted to Changzhou Hospital of Traditional Chinese Medicine from February 2020 to February 2022 were selected in this study.They were randomly divided into control group(n=41)and observation group(n=42)according to random number table method.The patients were treated with biofeedback for 4 weeks in both groups,and self⁃made Runchangling was additionally given to the patients in the observation group.The curative effect,TCM symptom scores,anal dynamics indicators,defecation time,defecation interval,72⁃hour defecation frequency,and the recurrence of FOOC within 3 months after treatment were compared between the two groups.Results The response rate of the observation group was higher than that of the control group(92.86%[39/42],73.17%[30/41],P<0.05).The toaditional chinese medicine(TCM)main symptom score,secondary symptom score and total TCM symptom score were decreased after treatment in both groups,and these scores in the observation group were lower than those in the control group(all P<0.05).The anal maximal contraction pressure(AMCP)and anal rest pressure(ARP)were decreased after treatment in both groups,and the duration of anal canal extrusion was shortened.The above⁃mentioned three parameters in the observation group were lower than those in the control group(all P<0.05).The defecation time and defecation interval in the observation group were less than those in the control group,and the number of defecation within 72 hours in the observation group was more than that in the control group(all P<0.05).The recurrence rate of the observation group was lower than that of the control group within 3 months after treatment(9.52%[4/42],26.83%[11/41],χ2=4.196,P=0.041).Conclusion Self⁃made Runchangling combined with biofeedback therapy can improve TCM symptoms and anal dynamics indicators,shorten defecation in
