卵巢囊肿剥离术后分别应用电凝止血和缝合止血的效果对比

摘　　要：目的:比较卵巢囊肿剥离术后电凝止血与缝合止血结合皮下注射促性腺激素释放激素类似物(GnRH-a)治疗卵巢巧克力囊肿的疗效。方法:回顾性分析我院2022年1月—12月80例卵巢巧克力囊肿患者的临床资料,均行卵巢囊肿剥离术,且术后皮下注射GnRH-a,依据术后止血方式进行分组,分为A组(缝合止血,40例)、B组(电凝止血,40例)。所有患者在术后应用亮丙瑞林缓释微球治疗。比较两组手术指标、痛经症状评分、盆腔疼痛症状评分、治疗有效率、不良反应。结果:B组手术时间低于A组(P<0.05),而B组失血量与A组比较,差异无统计学意义(P>0.05);B组治疗后痛经症状评分低于A组(P<0.05);B组治疗后盆腔疼痛评分、治疗有效率、围绝经期症状、过敏皮炎、肝肾功能异常、阴道不规则出血发生率与A组比较,差异无统计学意义(P>0.05)。A组术后1个月、6个月、12个月E2水平均高于B组,LH水平均低于B组(P<0.05);A组术后1个月FSH水平低于B组(P<0.05)。A组术后1个月、6个月、12个月F0均高于B组,A组术后1个月、6个月EDV均高于B组(P<0.05)。结论:缝合止血与电凝止血在卵巢囊肿切除术联合GnRH-a治疗卵巢巧克力囊肿患者中疗效相似,两者各有优势,其中电凝止血能缩短手术时间,减轻患者的痛经症状;缝合止血对卵巢功能影响小,能促进卵巢功能恢复。Objective:To compare the efficacy of electrocoagulation hemostasis and suture hemostasis combined with subcutaneous injection of gonadotropin-releasing hormone analogue(GnRH-a)in the treatment of ovarian chocolate cyst after ovarian cyst dissection.Methods:The clinical data of 80 patients with ovarian chocolate cyst in our hospital from January to December 2022 were retrospectively analyzed.All patients underwent ovarian cyst dissection and subcutaneous injection of GnRH-a after operation.According to the postoperative hemostasis method,they were divided into group A(suture hemostasis,40 patients)and group B(electrocoagulation hemostasis,40 patients).All patients were treated with leuprorelin sustained-release microspheres after operation.The surgical indexes,dysmenorrhea symptom score,pelvic pain symptom score,treatment efficiency and adverse reactions were compared between the two groups.Results:The operation time of group B was lower than that of group A(P<0.05).There was no significant difference in blood loss between group B and group A(P>0.05).The dysmenorrhea symptom score of group B was lower than that of group A after treatment(P<0.05).There was no significant difference in pelvic pain score,effective rate of treatment,perimenopausal symptoms,allergic dermatitis,abnormal liver and kidney function and irregular vaginal bleeding between group B and group A(P>0.05).he levels of E2 in group A were higher than those in group B at 1 month,6 months and 12 months after operation,and the levels of LH were lower than those in group B(P<0.05).The level of FSH in group A was lower than that in group B at 1 month after operation(P<0.05).F0 in group A was higher than that in group B at 1 month,6 months and 12 months after operation,and EDV in group A was higher than that in group B at 1 month and 6 months after operation(P<0.05).Conclusion:Suture hemostasis and electrocoagulation hemostasis are similar in the treatment of ovarian chocolate cyst patients with ovarian cyst resection combined with GnRH-a.Both have their
