不同剂量地奥司明治疗原发性下肢静脉曲张的效果

摘　　要：目的:探讨不同剂量地奥司明在原发性下肢静脉曲张治疗中的效果和安全性。方法:回顾性分析我院门诊2020年12月—2022年3月收治的71例原发性下肢静脉曲张患者作为研究对象。根据治疗方案的不同将患者分为观察组(n=37)和对照组(n=34)。两组患者均采用地奥司明治疗,观察组采用高剂量用药(0.9 g/次,2次/d),对照组采用低剂量用药(0.45 g/次,2次/d),比较两组治疗效果和药物不良反应发生率。结果:观察组治疗后CEAP分级Ⅰ级比例高于对照组,Ⅴ级比例低于对照组,差异有统计学意义(P<0.05);观察组治疗后2周凝血酶原时间、凝血酶时间、活化部分凝血活酶时间均高于对照组,差异有统计学意义(P<0.05);观察组治疗后2周Kruppel样转录因子2水平高于对照组,纤溶酶原激活物抑制剂-1水平低于对照组,差异有统计学意义(P<0.05);两组药物不良反应发生率比较,差异无统计学意义(P>0.05)。结论:地奥司明治疗原发性下肢静脉曲张的效果确切,且高剂量用药效果优于低剂量用药,且药物剂量的提升不会增加药物不良反应发生风险。Objective:To investigate the efficacy and safety of different doses of diosmin in the treatment of primary varicose veins of lower extremities.Methods:A retrospective analysis of 71 patients with primary varicose veins of lower extremities admitted to our hospital from December 2020 to March 2022 was conducted.According to the different treatment options,the patients were divided into observation group(n=37)and control group(n=34).The patients in the two groups were treated with diosmin.The observation group was treated with high dose(0.9 g/time,2 times/d),and the control group was treated with low dose(0.45 g/time,2 times/d).The therapeutic effect and the incidence of adverse drug reactions were compared between the two groups.Results:After treatment,the proportion of CEAP grade I in the observation group was higher than that in the control group,and the proportion of grade V was lower than that in the control group,and the differences were statistically significant(P<0.05).The prothrombin time,thrombin time and activated partial thromboplastin time in the observation group were higher than those in the control group at 2 weeks after treatment,and the differences were statistically significant(P<0.05).The level of Kruppel-like transcription factor 2 in the observation group was higher than that in the control group at 2 weeks after treatment,and the level of plasminogen activator inhibitor-1 was lower than that in the control group,the differences were statistically significant(P<0.05).There was no significant difference in the incidence of adverse drug reactions between the two groups(P>0.05).Conclusion:The effect of diosmin in the treatment of primary varicose veins of lower extremities is definite,and the effect of high dose is better than that of low dose,and the increase of drug dose will not increase the risk of adverse drug reactions.
