加味桑菊饮联合奥司他韦治疗乙型流感并发儿童急性良性肌炎的疗效分析

摘　　要：目的探讨加味桑菊饮联合奥司他韦治疗乙型流感并发儿童急性良性肌炎(benign acute childhood myositis,BACM)的临床效果。方法选取2021年1月至2024年1月于温州市中西医结合医院儿科住院的60例乙型流感并发BACM患儿,根据随机数字表法将其分为对照组和研究组,每组各30例。对照组患儿给予奥司他韦治疗,研究组患儿给予加味桑菊饮联合奥司他韦治疗。比较两组患儿的临床疗效、症状改善时间和治疗前后天冬氨酸转氨酶(aspartate transaminase,AST)、丙氨酸转氨酶(alanine transaminase,ALT)、肌酸激酶(creatine kinase,CK)、肌酸激酶同工酶(creatine kinase isozyme,CK-MB)、乳酸脱氢酶(lactate dehydrogenase,LDH)、肌红蛋白(myoglobin,Mb)水平。结果治疗后,研究组患儿的总有效率显著高于对照组(χ^(2)=5.455,P=0.020);研究组患儿的退热时间、腿痛缓解时间均显著短于对照组(P<0.05)。治疗5d后,两组患儿的AST、CK、CK-MB、LDH、Mb水平均显著低于本组治疗前(P<0.05),研究组患儿的AST、CK、CK-MB、LDH、Mb水平均显著低于对照组(P<0.05)。结论加味桑菊饮联合奥司他韦治疗可改善乙型流感并发BACM患儿的症状,降低肌酶和Mb水平,对患儿预后有积极作用。Objective To explore clinical effect of Jiawei Sangju decoction combined with oseltamivir in the treatment of influenza B complicated with benign acute childhood myositis(BACM).Methods Sixty children with influenza B complicated with BACM hospitalized in the Department of Pediatrics,Wenzhou Hospital of Integrated Traditional Chinese and Western Medicine from January 2021 to January 2024 were selected and divided into control group and study group according to random number table method,with 30 cases in each group.The control group was treated with oseltamivir,and the study group was treated with Jiawei Sangju decoction combined with oseltamivir.The clinical efficacy,symptom improvement time and aspartate transaminase(AST),alanine transaminase(ALT),creatine kinase(CK),creatine kinase isozyme(CK-MB),lactate dehydrogenase(LDH)and myoglobin(Mb)before and after treatment were compared between two groups.Results After treatment,the total effective rate of study group was significantly higher than that of control group(χ^(2)=5.455,P=0.020).The duration of fever and leg pain relief in study group were significantly shorter than those in control group(P<0.05).After 5 days of treatment,the levels of AST,CK,CK-MB,LDH and Mb in two groups were significantly lower than before treatment(P<0.05),and the levels of AST,CK,CK-MB,LDH and Mb in study group were significantly lower than those in control group(P<0.05).Conclusion Jiawei Sangju decoction combined with oseltamivir can improve the symptoms of influenza B with BACM,reduce the levels of muscle enzyme and Mb,and have a positive effect on the prognosis of children.
