摘 要:目的:比较LafosseⅠ型肩胛下肌损伤采用关节镜肩胛下肌修补术与肌清理术治疗的临床效果。方法:选取2019年1月—2022年10月重庆市忠县人民医院收治的50例LafosseⅠ型肩胛下肌损伤患者作为研究对象,根据治疗方法的不同将其分为研究组(n=25)与对照组(n=25)。对照组给予关节镜肩胛下肌清理术治疗,研究组给予关节镜肩胛下肌修补术治疗,比较两组患者疼痛程度[视觉模拟评分(VAS)]、肩关节功能[美国肩肘外科学会肩关节评分(ASES)、美国加州大学肩关节评分(UCLA)]、肩关节活动度及并发症。结果:术后6个月,两组患者被动活动、静息VAS评分降低,ASES、UCLA评分提高,外旋、内旋、前屈角度增大,但研究组与对照组比较,差异无统计学意义(P>0.05);研究组并发症发生率(4.00%)与对照组(12.00%)比较,差异无统计学意义(P>0.05)。结论:关节镜肩胛下肌修补术与肌清理术用于治疗LafosseⅠ型肩胛下肌损伤,均能够减轻患者疼痛感,改善肩关节功能与活动度,具有较高的安全性。Objective:To compare the clinical efficacy of arthroscopic repair and muscle dissection for Lafosse type Ⅰ subscapular muscle injury.Methods:A total of 50 patients with Lafosse type Ⅰ subscapular muscle injury who were admitted in the hospital from January 2019 to October 2022 were selected,and divided into the study group(25 cases)and the control group(25 cases)by different treatment methods.The control group received arthroscopic subscapular muscle cleaning surgery,while the study group received arthroscopic subscapular muscle repair surgery.The pain levels[Visual Analog Scale(VAS)],Shoulder joint function[American Society of Shoulder and Elbow Surgery Shoulder Joint Score(ASES),University of California Shoulder Joint Score(UCLA)],shoulder joint range of motion,and complications of the two groups were compared.Results:6 months after surgery,the passive activity and resting VAS scores in both groups decreased.The angles of external rotation,internal rotation and anterior flexion increased.There were no statistically significant difference between the study group and the control group(P>0.05).There was no significant difference in the complication rate between the study group(4.00%)and the control group(12.00%)with statistically significant difference(P>0.05).Conclusion:Arthroscopic repair of the subscapular muscle and muscle dissection for the treatment of Lafosse type I subscapular muscle injury can reduce patient pain,improve shoulder joint function and range of motion,with high safety.
