摘 要:目的:探讨乙型肝炎病毒(HBV)感染者HBV-脱氧核糖核苷酸(DNA)载量与血小板参数、炎性因子的相关性。方法:回顾性分析2020年12月—2022年12月样本医院纳入的100例HBV患者临床资料,依据HBV-DNA载量检测结果将患者分成低HBV-DNA载量组(32例)、中HBV-DNA载量组(38例)和高HBV-DNA载量组(30例)。所有研究对象均进行血清γ-干扰素(IFN-γ)、白细胞介素-33(IL-33)、肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、超敏C反应蛋白(hs-CRP)、血小板(PLT)、血小板比容(PCT)、平均血小板体积(MPV)、血小板分布宽度(PDW)、大血小板比率(P-LCR)水平检测,比较各组上述指标的水平变化情况,并分析病毒载量与相关指标水平的相关性。结果:与低HBV-DNA载量组相比较,中HBV-DNA载量组与高HBV-DNA载量组血清IFN-γ、PLT、PCT水平均更低,且高HBV-DNA载量组低于中HBV-DNA载量组,差异均有统计学意义(F=199.689、129.357、242.560,P<0.05);与低HBV-DNA载量组相比较,中HBV-DNA载量组与高HBV-DNA载量组血清IL-33、TNF-α、IL-6、hs-CRP、MPV、PDW、P-LCR水平均更高,且高HBV-DNA载量组高于中HBV-DNA载量组,差异均有统计学意义(F=113.69、450.569、591.189、803.741、7.809、90.131、18.869,P<0.05);病毒载量与血清IFN-γ、PLT、PCT水平呈负相关(r=-0.326、-0.123、-0.231,P<0.05),与血清IL-33、TNF-α、IL-6、hs-CRP、MPV、PDW、P-LCR水平呈正相关(r=0.256、0.362、0.423、0.253、0.325、0.331、0.351,P<0.05)。结论:血清IFN-γ、PLT、PCT水平越低,病毒载量越高,血清IL-33、TNF-α、IL-6、hs-CRP、MPV、PDW、P-LCR水平越高,病毒载量越高,疾病预后情况越差。Objective:To explore the correlation of HBV-DNA load with platelet parameters and inflammatory factors in patients with hepatitis B virus(HBV)infection.Methods:A total of 100 patients with HBV were enrolled in this retrospective study from December 2020 to December 2022.According to the HBV-DNA load test results,the patients were divided into low HBV-DNA load group(32 cases),medium HBV-DNA load group(38 cases)and high HBV-DNA load group(30 cases).All subjects were tested by serum interferon-γ(IFN-γ),interleukin-33(IL-33),tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),high-sensitivity C-reactive protein(hs-CRP),platelet(PLT),platelet specific volume(PCT),mean platelet volume(MPV),blood platelet distribution width(PDW)and large platelet ratio(P-LCR)levels were detected,the changes of the indicators above in each group were compared,and the correlation between viral load and related indicators was analyzed.Results:Compared with the low HBV-DNA load group,the levels of serum IFN-γ,PLT and PCT in the medium HBV-DNA load group and the high HBV-DNA load group were lower(F=199.689、129.357、242.560,P<0.05).The high HBV-DNA load group was lower than the medium HBV-DNA load group compared with the low HBV-DNA load group.The serum levels of IL-33,TNF-α,IL-6,hs-CRP,MPV,PDW and P-LCR in medium HBV-DNA load group and high HBV-DNA load group were higher,and the high HBV-DNA load group was higher than the medium HBV-DNA load group(F=113.69、450.569、591.189、803.741、7.809、90.131、18.869,P<0.05).Viral load was negatively correlated with serum levels of IFN-γ,PLT,and PCT(r=0.256、0.362、0.423、0.253、0.325、0.331、0.351,P<0.05);and positively correlated with serum levels of IL-33,TNF-α,IL-6,hs-CRP,MPV,PDW,and P-LCR(r=0.256,0.362,0.423,0.253,0.325,0.331,0.351;P<0.05).Conclusion:The lower the levels of serum IFN-γ,PLT and PCT are,the higher the viral load is,the higher the levels of serum IL-33,TNF-α,IL-6,hs-CRP,MPV,PDW and P-LCR,and the higher the viral load is,the worse the prognosis of the disease is
