摘　　要：目的探讨右美托咪定(Dex)复合芬太尼和氟比洛芬酯用于剖宫产术后产妇自控静脉镇痛(PCIA)的效果。方法选取2023年1—10月在晋江市中医院择期行剖宫产术的100例初产妇进行回顾性分析,根据PCIA用药方案不同分为研究组(n=50)与对照组(n=50)。两组均于手术结束时连接并启动镇痛泵,镇痛泵配方:研究组为Dex 1.5μg/kg+芬太尼1.0 mg+氟比洛芬酯200 mg+格雷司琼6 mg,对照组为芬太尼1.0 mg+氟比洛芬酯200 mg+格雷司琼6 mg,两组药物均用0.9%氯化钠配制至110 ml。记录术后8 h(T1)、12 h(T2)、24 h(T3)、48 h(T4)的视觉模拟评分法(VAS)评分。记录术后2 d内下床活动率、镇痛满意度评分、不良反应发生情况。记录术后第一晚和术后第二晚的匹兹堡睡眠质量指数(PSQI)评分。结果研究组在T1、T2、T3、T4的VAS评分低于对照组,术后2 d内下床活动率高于对照组,镇痛满意度评分高于对照组,术后第一晚和术后第二晚的PSQI评分低于对照组,差异均有统计学意义(P<0.05)。两组不良反应总发生率比较,差异无统计学意义(P>0.05)。结论Dex复合芬太尼和氟比洛芬酯PCIA可明显减轻剖宫产术后疼痛,提高产妇镇痛满意度,改善术后短期睡眠质量。Objective To investigate the efficacy of Dexmedetomidine(Dex)combined with fentanyl and flurbiprofen axetil on patient-controlled intravenous analgesia(PCIA)for parturient after cesarean section.Methods A total of 100 primiparas who underwent elective cesarean section in Jinjiang Hospital of Traditional Chinese Medicine from January to October 2023 were selected and retrospectively analyzed.They were divided into the study group(n=50)and the control group(n=50)according to different PCIA medication schemes.At the end of the operation,the analgesia pump was connected and started in both groups.As for the prescription of analgesia pump,the study group was treated with Dex 1.5μg/kg+fentanyl 1.0 mg+flurbiprofen axetil 200 mg+granisetron 6 mg,while the control group was treated with fentanyl 1.0 mg+flurbiprofen axetil 200 mg+granisetron 6 mg.Both groups of drugs were prepared with 0.9%sodium chloride to 110 ml.Visual analogue scale(VAS)score was recorded at 8 h(T1),12 h(T2),24 h(T3)and 48 h(T4)after operation.The activity rate of getting out of bed within 2 days after operation,the satisfaction score of analgesia and the occurrence of adverse reactions(ADRs)were recorded.On the first night and the second night after operation,Pittsburgh sleep quality index(PSQI)scores were recorded.Results The VAS scores of the study group at T1,T2,T3 and T4 were lower than those of the control group,the activity rate of getting out of bed within 2 days after operation was higher than that of the control group,the score of analgesia satisfaction was higher than that of the control group,and the PSQI scores on the first night and the second night after operation were lower than those of the control group,all with statistically significant differences(P<0.05).There was no statistically significant difference in the overall incidence of ADRs between the two groups(P>0.05).Conclusion Dex combined with fentanyl and flurbiprofen axetil PCIA can obviously relieve postoperative pain,improve maternal analgesia satisfaction and increase shortt
