中频超声联合活血止痛膏治疗膝骨关节炎的临床效果

摘　　要：目的探讨中频超声促透联合活血止痛膏治疗膝骨关节炎(KOA)的临床疗效,并对人基质金属蛋白酶抑制因子-1(TIMP-1)及基质金属蛋白抑制酶-3(MMP-3)的影响。方法选取2022年3月至2023年2月来甘肃省第二人民医院骨科门诊及住院部就诊的80例KOA患者,按照随机数表法分为观察组和对照组,每组各40例。其中观察组将活血止痛中药外敷至所选穴位,使用中频超声电导仪照射局部治疗;对照组采用生理盐水制成的凝胶贴片联合中频超声促透治疗。每次30 min,4次/周,共治疗4周。通过视觉模拟评分法(VAS)评分、骨关节炎评分量表(HSS)评分、36条目简明健康量表(SF-36)评价治疗效果,酶联法(ELISA)分析并比较两组患者血清中TIMP-1及MMP-3的含量。结果治疗后,观察组总有效率、膝关节HSS评分、疼痛VAS评分、SF-36评分均明显优于对照组(P<0.05);治疗后,观察组血清中的TIMP-1含量增加,而MMP-3含量降低,结果均明显优于对照组,差异有统计学意义(P<0.05)。结论中频超声促透联合活血止痛膏能够有效改善KOA患者血清中TIMP-1及MMP-3的含量,改善膝关节功能,明显提升患者的生活质量。Objective To investigate the clinical efficacy of medium frequency ultrasound penetration promotion combined with Huoxue Zhitong Ointment in the treatment of knee osteoarthritis(KOA),and its impact on human tissue inhibitors of metalloproteinase-1(TIMP-1)and matrix metalloprotein inhibitory enzyme-3(MMP-3).Methods A total of 80 patients with KOA admitted to the outpatient and inpatient department of orthopedics of the Second People’s Hospital of Gansu Province from March 2022 to February 2023 w ere selected,and they were divided into the observation group(n=40)and the control group(n=40)according to the random number table method.In them,the observation group was treated with Chinese materia medica for promoting blood circulation and relieving pain to selected acupoints,and medium frequency ultrasonic conductivity instrument was used to irradiate local treatment.While the control group was treated with gel patch made of normal saline combined with medium frequency ultrasound penetration promotion.It was 30 minutes per session,4 times per week,for a total of 4 weeks of treatment.Visual analogue scale(VAS)score,hospital for special surgery(HSS)score and medical outcomes study 36-item short form health survey(SF-36)were used to evaluate the therapeutic efficacy,and the contents of TIMP-1 and MMP-3 in serum of the two groups of patients were analyzed and compared by enzymelinked immunosorbent assay(ELISA).Results After treatment,the overall response rate in the observation group was 87.5%,and the knee joint H SS score,pain VAS s core and SF-36 score were all s ignificantly superior to those in the control group(P<0.05).After treatment,the serum TIMP-1 content in the observation group increased,while the serum MMP-3 content decreased,and the results were all significantly better than those in the control group,with statistically significant differences(P<0.05).Conclusion Medium frequency ultrasound penetration promotion combined with Huoxue Zhitong Ointment can effectively improve the contents of TIMP-1 and MMP-3 in
