摘 要:目的 考察美司钠注射液与选定的20种静脉用药物在模拟Y型输液器给药时物理相容性。方法 美司钠注射液用0.9%氯化钠注射液(NS)或5%葡萄糖注射液(5%GS)调配至药物浓度为20 mg/ml,其他所有静脉用药物根据药品说明书调配至临床用药浓度。模拟Y型输液器给药,美司钠溶液分别与每种静脉用药物溶液按1:1比例混合于玻璃试管中,立即(0 h)、0.5 h、1 h、2 h、3 h和4 h考察各混合溶液物理性状变化、丁达尔效应、浊度与pH变化。4 h期间,混合溶液在任一时间产生浑浊、沉淀、颜色变化、气体或丁达尔效应,浊度与0 h比较变化≥0.5浊度单位(NTU)或pH与0 h比较变化≥10%,认为两静脉用药物物理不相容。结果 在测定的20种静脉用药物中,美司钠与阿昔洛韦混合溶液产生浑浊,4 h浊度与0 h比较变化>0.5 NTU;美司钠与其他19种静脉用药物混合溶液4 h内澄清,不产生浑浊、沉淀、颜色变化、气体或丁达尔效应,浊度与0 h比较变化<0.5 NTU,pH与0 h比较变化<10%。结论 通过Y型输液器,用NS或5%GS调配的美司钠与阿昔洛韦混合溶液物理不相容,美司钠与其他19种静脉用药物混合溶液物理相容。Objective To assess the physical compatibility of Mesna Injection during simulated Y-site administration with 20 selected intravenous drugs.Methods Mesna Injection solution was diluted in 0.9 % sodium chloride(NS) or 5 % glucose injection to drug concentration of 20 mg/ml.All other intravenous drugs were reconstituted and diluted to the range of concentrations used clinically according to manufacturers' recommendations.Y-site administrations were simulated by independently mixing Mesna solution with other tested drug solutions in glass tubes at a 1:1 ratio.Physical characteristics,Tyndall effect,the change of turbidity and pH were investigated immediately(at 0 h) and at 0.5 h,1 h,2 h,3 h,and 4 h after mixing.Medication mixtures were considered physically incompatible during the 4 h observation period if there was visual evidence of haze,precipitation,gas evolution,color change,a positive Tyndall test at any time or a turbidity change of ≥0.5 nephelometric turbidity units(NTU),or pH change ≥10 % compared to 0 h.Results Of the 20 intravenous drugs tested,the combination of Mesna and acyclovir formed haze and showed a change in turbidity of >0.5 NTU compared to 0 h after mixing.The mixed solution of Mesa and 19 other intravenous drugs clarified within 4 h without producing turbidity,precipitation,color change,gas or Tyndall effect.The change in turbidity compared to 0 h was less than 0.5 NTU,and the change in pH compared to 0 h was less than 10 %.Conclusion During simulated Y-site administration,Mesna is physically incompatible with acyclovir in both NS and 5 %GS,and other 19 intravenous drugs are found to be physically compatible with Mesna.
