摘 要:目的探讨脑电图联合磁共振(MRI)对不同分级小儿急性胆红素脑病(ABE)的诊断及预后预测价值。方法选取2021年1月至2023年1月在菏泽市牡丹人民医院新生儿科住院的80例ABE患儿为研究对象,均进行脑电图检测和MRI检测,计算G/P比值以及9个电极的功率谱值,采用受试者工作特征(ROC)曲线分析脑电图联合MRI对ABE的诊断及预后预测价值。结果脑电图对不同分级ABE的误诊例数为轻度5例,中度2例,重度3例;MRI对不同分级ABE的误诊例数为轻度2例,中度1例,重度1例;脑电图联合MRI对不同分级ABE的误诊例数为轻度1例。预后不良组G/P比值、O1、O2、T3、T4、C3、C4、Cz、Fp1、Fp2功率谱值均高于预后良好组(P<0.05);ROC曲线结果显示,脑电图联合MRI对ABE预后预测的曲线下面积高于单一检查。结论ABE患儿脑电图9个电极的功率谱值和MRI G/P比值变化明显,且均与临床分级和预后有关,联合检查可提高预后预测和分级诊断效能。Objective To explore the diagnostic and prognostic value of electroencephalogram combined with magnetic resonance imaging(MRI)for acute bilirubin encephalopathy(ABE)in children with different grades.Methods A total of 80 children with ABE who were hospitalized in the Department of Neonatology,Mudan People’s Hospital of Heze City from January 2021 to January 2023 were selected as the research objects.The electroencephalogram and MRI tests were performed on them,and the G/P ratio and power spectrum values of 9 electrodes were calculated.Receiver operating characteristic(ROC)curve was used to analyze the value of electroencephalogram combined with MRI in the diagnosis and prognosis of ABE.Results The number of misdiagnosis cases of ABE in different grades by electroencephalogram was 5 cases for mild,2 cases for moderate,and 3 cases for severe.The number of misdiagnosis cases of ABE in different grades by MRI was 2 cases for mild,1 case for moderate,and 1 case for severe.The number of misdiagnosis cases of ABE in different grades by electroencephalogram combined with MRI was 1 case for mild.The G/P ratio,O1,O2,T3,T4,C3,C4,Cz,Fp1,and Fp2 power spectrum values of poor prognosis group were higher than those of good prognosis group(P<0.05).ROC curve results showed that the area under the curve of electroencephalogram combined with MRI in predicting ABE prognosis was higher than that of single examination.Conclusion The power spectrum values of 9 electrodes and MRI G/P ratio of ABE children change significantly,and all of them are related to clinical grade and prognosis.Combined detection can improve prognosis prediction and grading diagnosis efficiency.
