彩色多普勒超声、CTV与下肢穿静脉功能不全临床分级的关系分析

摘  要:目的分析彩色多普勒超声(CDU)、计算机断层扫描静脉造影(CTV)与下肢穿静脉功能不全(IPV)临床分级的关系。方法选取2021年2月~2023年1月在本院接受治疗的79例下肢IPV患者作为研究对象,所有患者均在术前行CDU、CTV检查,以手术结果作为金标准,分析CDU、CTV对下肢IPV的诊断价值。另按照CEAP分级将下肢IPV患者分为C1-C2组、C3-C4组、C5-C6组,比较不同临床分级患者CDU、CTV各项参数,采用Pearson法分析CDU、CTV各参数与下肢IPV临床分级的关系。结果手术结果显示79例患者共检出235条IPV,CDU、CTV诊断下肢IPV的准确率分别为97.47%(77/79)、98.73%(78/79),下肢IPV数目的检出率分别为95.74%(225/235)、97.87%(230/235),二者比较均无统计学差异(P>0.05);CDU和CTV检查IPV数目、内径比较均无统计学差异(P>0.05);C5-C6组IPV数目、内径、流速、返流时间、图像质量评分均高于C3-C4组和C1-C2组(P<0.05),C3-C4组均高于C1-C2组(P<0.05);Pearson分析结果显示,IPV数目、内径、流速、返流时间、图像质量评分与下肢IPV均呈正相关(P<0.05)。结论CDU、CTV对下肢IPV均具有较好的诊断价值,且IPV数目、血管内径、反流速度、返流时间等参数均与CEAP分级密切相关,对下肢IPV患者病情评估和治疗有一定指导意义。Objective To analyze the relationship between color doppler ultrasound(CDU),computed tomography venography(CTV)and clinical grading of the lower extremity functional insufficiency of perforating veins(IPV).Methods 79 patients with lower limb IPV who received treatment in our hospital from February 2021 to January 2023 were selected as the research subjects,and all patients underwent CDU,CTV examinations.The diagnostic value of CDU and CTV for lower limb IPV was analyzed based on the surgical results as the gold standard.According to CEAP grading,the patients with lower limb IPV were divided into C1-C2 group,C3-C4 group,and C5-C6 group,and the CDU and CTV parameters of patients with different clinical grading were compared.The Pearson method was used to analyze the relationship between CDU,CTV parameters and clinical grading of lower limb IPV.Results The surgical results showed that a total of 235 IPVs were detected in 79 patients,the accuracy rates of CDU and CTV in diagnosing lower limb IPV were 97.47%(77/79)and 98.73%(78/79),respectively,and the detection rates of the number of lower limb IPVs were 95.74%(225/235)and 97.87%(230/235),respectively,and there was no statistical difference between the two groups(P>0.05).There was no statistically significant difference in the number and inner diameter of IPVs between CDU and CTV examinations(P>0.05).The number,inner diameter,flow rate,reflux time,and image quality score of IPV in C5-C6 group were higher than those in C3-C4 group and C1-C2 group(P<0.05),while C3-C4 group was higher than C1-C2 group(P<0.05).Pearson analysis results showed that the number,inner diameter,flow velocity,reflux time,image quality score,and lower limb IPV were positively correlated(P<0.05).Conclusion CDU and CTV have good diagnostic value for lower limb IPV,and their parameters such as IPV number,vascular diameter,reflux velocity,and reflux time are closely related to CEAP grading,which has certain guiding significance for the evaluation and treatment of lower limb IPV patients.
