摘  要:目的:分析带针胸管胸腔闭式引流术联合高频机械通气(HFOV)治疗新生儿气胸的临床效果及安全性。方法:选取2022年1月—2023年12月昆明市第一人民医院收治的新生儿气胸患儿62例为研究对象,随机分为对照组和观察组,各31例。对照组实施传统胸腔闭式引流术联合常频机械通气治疗,观察组实施带针胸管胸腔闭式引流术联合HFOV治疗。比较两组治疗效果、血气分析指标、治疗指标及并发症发生率。结果:观察组治疗总有效率高于对照组,差异有统计学意义(P=0.006);治疗12 h后,两组动脉血氧分压、动脉血二氧化碳分压、氧合指数、吸入氧浓度优于治疗前,观察组优于对照组,差异有统计学意义(P<0.05);观察组操作时间、起效时间、呼吸机参数下调时间、胸部X线恢复正常时间、住院时间短于对照组,伤口直径小于对照组,差异有统计学意义(P<0.05);观察组并发症发生率低于对照组,差异有统计学意义(P=0.005)。结论:带针胸管胸腔闭式引流术联合HFOV治疗新生儿气胸的效果显著,可改善新生儿血气分析指标,缩短病情转归时间,安全性较高。Objective:To analyze the clinical effect and safety of closed chest drainage with needle chest tube combined with high�frequency mechanical ventilation(HFOV)in the treatment of neonatal pneumothorax.Methods:Sixty-two children with neonatal pneumothorax admitted to Kunming First People's Hospital from January 2022 to December 2023 were selected for the study,and were randomly divided into the control group and the observation group,each with thirty-one cases.The control group was treated with traditional closed chest drainage combined with constant-frequency mechanical ventilation,and the observation group was treated with closed chest drainage with needle chest tube combined with HFOV.Compare the two groups'treatment effect,blood gas analysis index,treatment index and complication rate.Results:The total effective rate of treatment in the observation group was higher than that in the control group,and the difference was statistically significant(P=0.006);after 12 h of treatment,the arterial blood partial pressure of oxygen,arterial blood partial pressure of carbon dioxide,oxygenation index,and concentration of inhaled oxygen of the two groups were better than that before treatment,and the observation group was better than that of the control group,and the difference was statistically significant(P<0.05);the operation time,onset time,ventilator parameter down time,chest X-ray return to normal time,hospitalization time of the observation group was shorter than that of the control group,and the diameter of the wound was smaller than that of the control group,the difference was statistically significant(P<0.05);the complication rate of the observation group was lower than that of the control group,the difference was statistically significant(P=0.005).Conclusion:The effect of closed chest drainage with needle chest tube combined with HFOV in treating neonatal pneumothorax is remarkable,which can improve the indexes of blood gas analysis of neonates,shorten the time of regression,and has high safety.
