加味五味消毒饮联合抗生素骨水泥治疗下肢感染性创面的临床效果观察

摘　　要：目的:观察加味五味消毒饮联合抗生素骨水泥治疗下肢感染性创面的效果。方法:选取2022年12月—2024年4月仁怀市中医院收治的下肢感染性创面患者90例作为研究对象,采用随机数字表法分为观察组与对照组,各45例。对照组采用抗生素骨水泥治疗,观察组在对照组基础上采用加味五味消毒饮治疗。比较两组患者疼痛评分、实验室检查指标、治疗效果及细菌培养转阴率。结果:术后1、3、12 d,两组视觉模拟评分法评分逐渐降低,术后3、12 d,观察组低于对照组,差异有统计学意义(P<0.05);术后12 d,两组红细胞沉降率、C反应蛋白、降钙素原水平低于术前,观察组低于对照组,差异有统计学意义(P<0.05);观察组治疗总有效率高于对照组,差异有统计学意义(P=0.026);观察组创口分泌物细菌培养转阴率高于对照组,差异有统计学意义(P=0.024)。结论:加味五味消毒饮联合抗生素骨水泥治疗下肢感染性创面效果较好,可减轻疼痛,改善实验室指标,促进创口分泌物细菌培养转阴。Objective:To observe the effect of modified Wuwei disinfection decoction combined with antibiotic bone cement on infected wounds of lower limbs.Methods:A total of ninety patients with lower limb infectious wounds admitted to Renhuai Hospital of Traditional Chinese Medicine from December 2022 to April 2024 were selected as the study objects,and were divided into observation group and control group by random number table method,with forty-five cases in each group.The control group was treated with antibiotic bone cement,and the observation group was treated with modified Wuwei disinfection decoction on the basis of control group.The pain score,laboratory test index,therapeutic effect and negative conversion rate of bacterial culture were compared between the two groups.Results:On the 1st,3rd and 12th day after operation,the score of visual analogue scoring in the two groups gradually decreased,and on the 3rd and 12th day after operation,the observation group was lower than the control group,the difference was statistically significant(P<0.05);12 days after surgery,erythrocyte sedimentation rate,C�reactive protein and procalcitonin levels in two groups were lower than before surgery,and observation group was lower than control group,the difference was statistically significant(P<0.05);the total effective rate of the observation group was higher than that of the control group,and the difference was statistically significant(P=0.026);the rate of bacterial culture of wound secretion turning negative in the observation group was higher than that in the control group,and the difference was statistically significant(P=0.024).Conclusion:Modified Wuwei disinfection decoction combined with antibiotic bone cement has a good effect in the treatment of infected wounds of lower limbs,which can reduce pain,improve laboratory indexes and promote the bacterial culture of wound secretions to turn negative.
