摘 要:目的:分析MRI诊断肩胛下肌(SSC)腱撕裂的价值。方法:选取2022年7月—2023年4月玉林市中西医结合骨科医院收治的疑似SSC腱撕裂患者60例为研究对象,均进行MRI和肩关节镜检查,以肩关节镜检查结果作为“金标准”,分析MRI诊断结果、各分型检出情况。结果:肩关节镜结果显示,疑似SSC腱撕裂患者60例中,确诊SSC腱撕裂41例,占比68.33%。MRI检查出SSC腱撕裂40例,占比66.67%。两种检查方式SSC腱撕裂检出率比较,差异无统计学意义(P>0.05)。其中,MRI对4型、5型SSC腱撕裂诊断准确率为100.00%。肩关节镜与MRI诊断结果一致性很强(Kappa=0.849,P<0.001)。结论:MRI诊断SSC腱撕裂的临床价值较高,能检出各分型,尤其适用于诊断4型、5型SSC腱撕裂,能为后续手术选择和治疗方案制定提供参考。Objective:To analyze the value of MRI in diagnosing subscapular muscle(SSC)tendon tear.Methods:Sixty patients with suspected SSC tendon tear admitted to Yulin Orthopedic Hospital of Integrated Traditional Chinese and Western Medicine from July 2022 to April 2023 were selected as the study objects,all of whom underwent MRI and shoulder arthroscopy.The results of shoulder arthroscopy were taken as the"gold standard"to analyze the MRI diagnosis results and the detection of each type.Results:The results of shoulder arthroscopy showed that among the 60 suspected patients with SSC tendon tear,41 cases were diagnosed with SSC tendon tear,accounting for 68.33%;MRI showed that 40 patients with SSC tendon tear,accounting for 66.67%;there was no significant difference in the detection rate of SSC tendon tear between the two methods(P>0.05);among them,the accuracy of MRI diagnosis of tendon tears in type 4 and type 5 SSC was 100.00%;there was a strong consistency between shoulder arthroscopy and MRI diagnosis(Kappa=0.849,P<0.001).Conclusion:MRI has high clinical value in the diagnosis of SSC tendon tears,and can detect various types,especially for the diagnosis of type 4 and type 5 SSC tendon tears,and can provide reference for subsequent surgical selection and treatment plan formulation.
